Job summary

An exciting opportunity has arisen for a Band 5RMN Nurse to join Evesham Neighbourhood Community Mental Health Teams (NMHT).

Contributing to the delivery of ourservices, facilitating group interventions and working with individuals onimproving the functioning in their lives, as a key member of our MDT across thePrimary Care Networks ensuring our patients receive expert care focused onrecovery.

The post is for hours a week Monday to Friday

Main duties of the job

Use their initiative and take responsibility for themselves andthe quality of their work and the service they provide to patients. Be responsible for assessing care needs, developing, implementing,commissioning and evaluating programmes of care under the supervision of anominated more senior professional. Under the supervision of nominated senior professional, managingand carrying continuing responsibility for a defined caseload of patients withcomplex mental health needs, taking on the role of care co-ordinating as theCPA care co-ordinator.

Job description

Job responsibilities

Under the direction of a nominated supervising professional:

Provide specific PsychiatricNursing interventions to allocated patients/clients and their relatives who arereferred to the service. Work in conjunction with allprimary care services and partnership agencies to foster closemultidisciplinary and multi-agency working. Participate in regular supervision withnominated supervisor and evaluate caseload. Assess care needs, develop,implement and evaluate programs of care utilising the Recovery model.

Clinical

Function as an active member of the CommunityAssessment Recovery Service. Ensure high standards patient care, ensuringpatients individuality and dignity is maintained at all times, while promotingindependence. Maintain up to date, accuraterecords within Trust and NMC guidelines and keep these confidential and in asafe place. Responsible for ensuring riskassessments are carried out in conjunction with other agencies. Ensure that the philosophy of Person-CentredCare is upheld at all times. Fully implement the agreed care plan toinclude commissioning where required, with the support of more senior healthand social care staff and ensure that it is regularly reviewed and updated. Ensure that the requirements of the MentalHealth Act 1983 are met and that patients rights under the Act are upheld. Participate in all aspects ofdischarge and aftercare including co-operation with Section 117 meetings andthe Care Programme Approach and Care Co-ordination. Ensure effective liaison with allagencies to ensure seamless service provision. Ensure that information regardingpatients needs and progress are communicated promptly and accurately to otherteam members involved in their care, having due regard for confidentiality. Facilitate open and harmoniouscommunications amongst staff within the team and with other departments withthe aim of promoting multidisciplinary approaches to care. Participate with direct support of a seniorcolleague in undertaking the role of Professional Cover. Ensure that each patient inhis/her care receives a comprehensive multidisciplinary CPA assessment in linewith the CPA process and is the subject of a regular updated care plan which isresponsive to changing needs. Where practicable, this care plan must beformulated in conjunction with the patient.

1 years post registration experience on an acute psychiatric ward or Post registration experience working with Adults in Mental Health

Desirable

Experience of Solution Focussed Approach and / or Cognitive Behavioural Therapy Experience of Counselling skills (Bereavement and Loss).
