Job summary

The Evesham, Broadway, Bredon and Inkberrow (EBBI) NeighbourhoodTeam is a friendly and hardworking team who are looking for an enthusiastichealthcare assistant to join us at our base in Evesham.

The Neighbourhood team is looking to recruit someone who is happyto work across our whole patch on a full-time basis ( hours), and who iskeen to join the team in supporting us to deliver high quality care.

We are looking for a passionate individual who is excited bychange and keen to help shape a new way of delivering community integratedcare. We work closely with our GPs andwork hard at developing relationships with primary care services, drawing onthe skills of our MDT colleagues to manage complex patients and ensure we are responsive and patient centred.

Main duties of the job

While on duty you will either be working aloneor alongside colleagues to support delegated tasks in line with Trust standardsand core values.

Each task will have been assessed by aqualified Healthcare professional and a care plan will be in place for you tofollow and report back on.

Tasks may include personal care, supportingdaily activities, participating in manual handling and other allocated taskssuch as dressing wounds, setting up equipment, and recording observations.

All clinical documentation is recorded in linewith Trust guidelines, and you will give daily handovers to team leads summarisingyour involvement with your allocated patients.

Clear and timely Communication is key to yourrole which may be in a verbal, non-verbal or written format to support patientcentred care.

There are three core working shifts that cover the service from 7:30am-6:30pm, Monday to Sunday.

The post holder will:

Maintaining accurate and contemporaneousrecords in accordance with Trust policies/procedures, while always maintainingconfidentiality.

Provide a high standard of outcome orientatedcare and support following a care/recovery plan under the supervision andguidance of qualified health care professionals.

Person Specification

Knowledge

Essential

Basic knowledge of physical/mental health conditions. A level of understanding of rehabilitation.

Experience

Essential

Experience of working with older adults in a health/social care setting

Desirable

Community experience of older adults with physical and/or mental health problems Experience of working and liaising with staff from different disciplines

Qualifications

Essential

Level 3 qualification ( NVQ 3, A-level) in Care Or Level 2 qualification ( NVQ2, GCSE Grade A-C) in Care and will be expected to undertake the relevant Level 3 qualification on appointment to post Will be expected to meet the Care Certificate standards within 12 weeks of commencing employment with the Trust I understand that I will be asked to provide original certificates to verify my qualifications at point of offer
