Job summary

We are looking for a full time, hours, enthusiastic teamplayer to come and join our nursing team. Evesham Community Hospital is a 50 beddedstroke and general rehabilitation unit. The nursing staff work alongside thetherapy team and adopt a multidisciplinary team approach to providing rehab tothe patients.

We require an innovative, enthusiastic, motivated individualinterested in all areas of rehab. You will be required to plan, implement,evaluate and take responsibility for the nursing care of the ward patients on adaily basis. You will be expected to facilitate and support the patient andtheir families throughout their pathway from admission to discharge.

Evesham Community Hospital plays a critical role in thedelivery of local NHS services, providing more care and treatment closer topeoples homes. Emphasis is to reduce length of stay whilst ensuring excellentcare which is safe, effective, personalised and compassionate and to thestandards you would expect for your family and friends.

Main duties of the job

As aStaff Nurse you will be responsible for providing care to people admitted toour wards for rehabilitation. We work closely as a multi-disciplinary team. Ourfocusispromoting the comfort and wellbeing of patients,enablingthemto live as independently as possible. Evesham is a friendly team to work with. As ateam we encourage ongoing learning and development through reflection andclinical supervision which we try and embed in our daily practice. Our nursesare required to facilitate the day to day running of the ward, administeringmedications, working alongside all members of the MDT as well as patients,their families and carers to provided 24/7 rehabilitation to enable the highestlevel of independence and quality of life. Our nurses are required to attend meetings asneeded such as MDT and CHC or Discharge Planning.

Job description

Job responsibilities

Responsible for theassessment, planning, implementation and evaluation of patients care withinthe Ward/unit, working in accordance with the philosophy of patient centeredcare and to NSF and other Trust standards. Practice as a memberof the Community Hospital Nursing team, to support and work in partnership withthe wider multidisciplinary team. Planning andDelivery of Individual Nursing Care under Direction of Senior Staff Work within the NMCCode of Professional Conduct. Assess, plan, and,implement and evaluate patient focused nursing care. Document care givenand maintain contemporaneous records. Maintain records inaccordance with the Trusts policy and that of NMC Code of ProfessionalConduct. Assist inimplementation of health promotion strategies, as agreed by Ward Manager. In consultation withsenior qualified staff, provide and monitor patient centered care within Trustagreed standards. In collaborationwith other team members organise the reception and discharge of patientsensuring effective communication in transfer of information. Carry out delegatednursing care. Use and participatein research / audit in care delivery. Adhere to andpromote all clinical practice, health and safety and corporate Trust Policiesand Guidelines.
