Job summary

28 hrs week

To provide administrative support to GPs, Nurses, HCAs, Administration staff and wider community teams.

Key roles of Reception/telephony duties, care navigation, pathology results, hospital letters and task list/messages. Along with notes summarising in time the role will develop to include child health surveillance (tracking childhood immunisations and also supporting the Safeguarding lead with administrative tasks).

The role will involve working to ensure that the surgerys appointments, systems and processes are as effective as possible to allow us to deliver an outstanding level of personal medical services to the patients of the practice.

Main duties of the job

Data Support Officer . There may be, on occasion, arequirement to carry out other tasks; this will be dependent upon factors suchas workload and staffing levels:

Other tasks will include Ensure an effective andefficient reception service is provided to patients and any other visitors to the practice

Process personal andtelephone requests for appointment, visits and telephone callsaccording to practice protocols and guidelines

Receive, divert and make telephonecalls as required. Take messages.

Using judgment andcommunication skills ensure that patients with no prior appointment but whoneed urgent consultation are seen in a logical and non-disruptive manner.

Processing and distributingincoming (and outgoing) mail

Preparationof morning visits in liaison with GPs

Providesupport to reception/telephony clerks by dealing with any patient queriesrequiring further investigation

Providesupport to GPs with making telephone calls on their behalf

Data entry andonline referral

Providesupport to the GPs and Assistant Practice Manager in relation to chasinghospital appointments /queries

Processingof sick note requests

Liaisingwith community Nursing team, Care Co-Ordinators etc

Liaisingwith Coroner, medical examiner, registry office and funeral homes followingdeaths

Arrangeappointments for medicals as agreed with GP

Arrangeweekly ward rounds for care home

We have 7 GPs, 5 in the nursing team and a whole host of additional clinical roles and a fully comprehensive administrative team.

Job responsibilities

Deal with all generalenquiries

Process personal andtelephone requests for appointment, visits and telephone consultationsaccording to practice protocols and guidelines

Liaisingwith community Nursing team, Care Co-Ordinators etc

Dailymessages for GPs

Liaisingwith Coroner, medical examiner, registry office and funeral homes followingdeaths

Arrangeappointments for medicals that are required and agreed with GP

Arrangeweekly ward rounds for care home residents

Tobe pro active in making arrangements DNS1 preparation of paperwork forcomm nursing team to administer medications accordingly

Firstpoint of contact for palliative, elderly and vulnerable patients and theirfamilies

Provideemotional support

Toundertake any other duties or responsibilities which may reasonably be required

Attendanceat month community health care team meetings (MDT)

Attendanceat end of life care meetings (GSF)

Triaging/assigninghome visits

Compilinglist for ward round

ArrangingDVLA medicals

Other responsibilities

To have a thorough knowledge of all practice procedures

To work in accordance to practice protocols

Ensure building security have thorough knowledge of doors/windows/alarms

Opening up/locking up of practice premises and maintaining security inaccordance with practice protocols

Computer data entry processing and recording information in accordance withpractice procedures

Providing clerical assistance to practice staff as required from time to time

Undertake any additional duties as requested by the partners or the practicemanager

Administration

Mail to patients and required, calls and reminders

Photocopy as requested

Medical record summarising:

To review medical records and produce an accurate summary of the patientsmedical history, within the Practices protocols and agreed timescales.

Enter Read Code information onto the Practices computer system.

Use medical knowledge to correct mistakes found in existing patient records.

Adhere to and monitor progress ofnotes summarising against Practice targets, maintaining structure and protocolswhen recording new information.

Be aware of specific Read Codes andassociated records; following Practice protocols for recording, obtaining andfiling patient records as appropriate child protection.

Assist GPs and other Practice staff with queries relating to medical records.

Liaise with internal/external departments or healthcare providers as require.

Check vaccinations arerecorded correctly on notes

Child health surveillance:

Trackingchildhood immunisations and reviews

Supportingthe Safeguarding lead with administrative tasks)

Arranging safeguarding meetings and taking minutes

Qualifications

Essential

Equivalent C or above O'level Maths and English. General Practice Experience. Hard working, reliable team player.
