Job summary

We are looking for a wellbeing practitioner to offer wellbeing interventions and support to children and young people as part of Open Minds Transition Support Services. The role will include working one evening a week with the potential of at least one additional evening a month to accommodate the facilitation of activities held for the young people receiving support from the service.

The Open Minds Transition Support Service (TSS) service is commissioned by the South West Yorkshire NHS Foundation Trust. It aims to improve mental wellbeing, build resilience and get young people into activities we know are good for their wellbeing through social prescribing and signposting.

The TSS Wellbeing Practitioner will manage a busy caseload and act as a key person for young people aged 16-25 years, delivering the highest quality service ensuring that young people being discharged from CAMHS and/or referred by other services around the Calderdale area have access to effective wraparound support in a timely and accessible manner. They will be responsible for managing a busy caseload of young people, offering emotional health & wellbeing interventions, signposting and referring internally/externally and managing administrative duties.

The Wellbeing Practitioner will be directly responsible to the TSS Service Manager and will have monthly clinical, case and management supervision.

Main duties of the job

To provide one to one wellbeing support sessions for young people and young adults on a weekly basis focusing on building emotional wellbeing and resilience.

To develop and facilitate workshops/activities on a monthly basis focusing on building emotional wellbeing and resilience.

To provide timely, effective and accessible support by offering strategies, resources and approaches which can support a child/young persons wellbeing over a short term period.

Providing support and collaboratively identifying, managing, mitigating and resolving problematic thoughts, feelings and behaviours.

Under supervision, undertake accurate assessments of risk to self and others

To signpost YPs for further support as and when necessary.

To manage a busy caseload of children/YPs.

To identify and report safeguarding incidents, in line with relevant Invictus Wellbeing Services policies.

Provision of information, advice and guidance (IAG) to YPs and staff regarding related issues, typically these would include general welfare, academic, professional, health and personal.

Reporting on trends and needs in the above areas in order to improve and develop support for young people.

To update monitoring documents/reports consistently and analyse trends and use them where appropriate

To keep spreadsheets, administrative processes and client records up to date.

Ensure that confidentiality is always protected in accordance with confidentiality policy

To manage busy caseloads ensuring effective communication

Invictus Wellbeing is West Yorkshires Children and Young Persons Mental Health Charity. We offer timely, effective and accessible mental health support whilst also promoting positive mental wellbeing in the communities we serve. Experience

Essential

2 Years experience working with children or young people. A track record of delivering results with quality outcomes, measured to key performance indicators Experience of building and nurturing strong relationships with service users.

Desirable

Experience of working within a mental health or community health and wellbeing setting

Qualifications

Essential

Is educated to a degree level/is currently studying.

Desirable

Possesses some formal training or qualifications in Counselling or Therapy. Holds a recognised membership of a relevant professional body (BACP/ PTUK/ UKCP/NCS/BPS)

Knowledge & Skills

Essential

Knowledge of mental health & community based approaches to supporting mental health and wellbeing. Demonstrates understanding of working with children and young people who require or would benefit from emotional health and wellbeing support Ability to manage a caseload and ensure that administrative duties are kept under control. A sound understanding of effective performance monitoring. . Numerate with an ability to understand, analyse and make effective use of data. Ability to build a rapport naturally which creates a team ethos and culture that represents the organisation culture and values.

Knowledge of relevant national policies, best practices, quality frameworks and local approaches to implementation within the specific area of mental health support for children and young people
