About the Role

We are seeking a highly skilled and experienced Mental Health Practitioner to join our team at North Halifax Primary Care Network. As a key member of our multidisciplinary Personalised Care Team, you will play a vital role in enhancing the mental health services we provide to our patients.

Key Responsibilities
  • Assess, diagnose, and treat patients with a range of mental health conditions, working independently and collaboratively with other healthcare professionals.
  • Initiate direct referrals to secondary and community services as needed.
  • Manage daily surgeries, including face-to-face consultations and telephone follow-ups.
  • Collaborate with GPs, ACPs, nursing staff, and other non-clinical support staff to provide comprehensive patient care.
  • Participate in implementing new working methods to manage increasing demand in primary care.
  • Provide education and training to other staff members.
  • Undertake research and contribute to the development of our services.
Requirements
  • Nursing and Midwifery Registered Mental Health Professional with current UK Professional Registration.
  • Minimum 3 years post-registration experience in a role matching the required clinical skills.
  • Evidence of continuing professional development.
  • Excellent communication skills, both written and verbal.
  • Ability to communicate difficult messages to patients and families.
  • Negotiation and conflict management skills.
  • In-depth therapeutic and clinical knowledge and understanding of evidence-based healthcare principles.
Desirable Qualifications and Experience
  • Non-medical Prescriber or Independent/Supplementary Nurse Prescribing Qualification.
  • Primary care or community nursing experience of quality initiatives such as clinical benchmarking audit experience.
  • Advanced communications skills.
  • Knowledge of SystmOne and Emis Web Clinical Systems.
Working Conditions

You may be required to travel independently between practice sites and attend meetings hosted by other agencies.
