We are seeking a highly skilled and experienced Mental Health Practitioner to join our team at North Halifax Primary Care Network. As a key member of our multidisciplinary Personalised Care Team, you will play a vital role in enhancing the mental health services we provide to our patients.
Key Responsibilities- Assess, diagnose, and treat patients with a range of mental health conditions, working independently and collaboratively with other healthcare professionals.
- Initiate direct referrals to secondary and community services as needed.
- Manage daily surgeries, including face-to-face consultations and telephone follow-ups.
- Collaborate with GPs, ACPs, nursing staff, and other non-clinical support staff to provide comprehensive patient care.
- Participate in implementing new working methods to manage increasing demand in primary care.
- Provide education and training to other staff members.
- Undertake research and contribute to the development of our services.
- Nursing and Midwifery Registered Mental Health Professional with current UK Professional Registration.
- Minimum 3 years post-registration experience in a role matching the required clinical skills.
- Evidence of continuing professional development.
- Excellent communication skills, both written and verbal.
- Ability to communicate difficult messages to patients and families.
- Negotiation and conflict management skills.
- In-depth therapeutic and clinical knowledge and understanding of evidence-based healthcare principles.
- Non-medical Prescriber or Independent/Supplementary Nurse Prescribing Qualification.
- Primary care or community nursing experience of quality initiatives such as clinical benchmarking audit experience.
- Advanced communications skills.
- Knowledge of SystmOne and Emis Web Clinical Systems.
You may be required to travel independently between practice sites and attend meetings hosted by other agencies.
