Adult Mental Health Practitioner

We are seeking a skilled and experienced Adult Mental Health Practitioner to join our team at South West Yorkshire Partnership NHS Foundation Trust. As a key member of our community mental health team, you will play a crucial role in delivering high-quality, recovery-focused care to individuals with complex mental health needs.
Key Responsibilities- Conduct comprehensive assessments and risk evaluations to identify individual needs and develop personalized care plans.
- Provide skilled interventions and support to individuals with mental health problems, promoting engagement, stability, and recovery.
- Work collaboratively with service users, carers, and other healthcare professionals to ensure seamless care coordination and leadership.
- Supervise and support other team members, promoting a culture of excellence and continuous learning.
- Develop and maintain accurate patient care records, adhering to professional codes of conduct and trust procedures.
- Participate in the formulation of policies and procedures at unit level, ensuring alignment with national and local practice standards.
- Current registration with the Nursing and Midwifery Council (NMC) or Social Work England/Health and Care Professions Council (HCPC).
- Experience of working with individuals with complex mental health needs, including those with dual diagnosis, peri-natal mental health, and personality disorders.
- Strong assessment and care planning skills, with the ability to identify and manage risk in a variety of settings.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- A current driving license and access to a car during working hours (reasonable adjustments will be considered for applicants with disabilities).
