Adult Mental Health Practitioner

We are seeking a skilled and experienced Adult Mental Health Practitioner to join our Community Mental Health Team in Calderdale. As a key member of the team, you will play a crucial role in delivering high-quality, recovery-focused care to individuals with complex mental health needs.
Key Responsibilities- Conduct comprehensive assessments and risk evaluations to identify individual needs and develop personalized care plans.
- Provide skilled interventions and support to individuals with mental health problems, promoting engagement, stability, and recovery.
- Work collaboratively with service users, carers, and other healthcare professionals to ensure seamless care coordination and leadership.
- Supervise and support other team members, promoting a culture of excellence and continuous learning.
- Maintain accurate patient records and contribute to the development of policies and procedures at the unit level.
- Current registration with the Nursing and Midwifery Council (NMC) or Social Work England/Health and Care Professions Council (HCPC).
- Experience of working with adults with complex mental health needs, including those with dual diagnosis, psychosis, and personality disorder.
- Strong assessment and care planning skills, with the ability to work holistically and bio-psycho-socially.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- A current driving license and access to a car during working hours (reasonable adjustments will be considered for applicants with disabilities).
