Job summary

This post is an exciting opportunity to join the Inpatient Rehabilitation Team at St Pancras Rehabilitation Unit.

You will be responsible for a providing specialist Neurological rehabilitation. You will work closely with the other AHP staff ( Neuro Physiotherapy Pathway Coordinator, General Rehabilitation Physiotherapist, Occupational Therapist and the wider Integrated Multidisciplinary Team.)

The inpatient multi-pathology and Neurological rehabilitation unit has 53 beds. The unit has strong links with community partners and the NCL Intermediate Care bed base.

The MDT and Occupational Therapy team embrace innovative evidence based practice. As a service we work in partnership with patients and their families /carers and encourage self - management activities at all stages in the patient journey. You will benefit from the experience and support offered by this highly integrated service. You will support and deliver intensive rehabilitation to patients with both Level 2 (6 beds) and Level 3 (17 beds) rehabilitation needs. We can offer opportunities to participate in clinical research projects and service development.

The unit has established training programmes in place and there are frequently opportunities for all staff to develop knowledge and skills within their role.

Main duties of the job

As an NHS Trust we strongly encourage and support vaccination as this remains the best way to protect yourself, your family, your colleagues and of course patients and service users when working on our healthcare settings.

To manage a complex neurological caseload using evidence based clinical reasoning, critical thinking, reflection, and analysis to support assessments, goal setting, personalised rehabilitation and disability management. To work flexibly with all members of the team to deliver an integrated rehabilitation service to clients in accordance with the philosophy and operational policies of the team. At times this will include working with a small number of patients with general rehabilitation needs ( patients with frailty, falls, amputee) in addition to a primary caseload of level 2 and level 3 patients requiring neurological rehabilitation. To support maintain and further develop a high quality, efficient and effective Occupational Therapy Service within a multidisciplinary context and in accordance with the philosophy and operational policies for the Organisation. To be responsible for the supervision and monitoring of junior staff, including Band 5 OT's, Rehabilitation Assistants and OT students. Cross-disciple working to assist with supervision and monitoring of staff from other professions may be required. To actively engage in a wide range of Continuing Professional Development activities.

You will lead on manual handling, pressure care and equipment needs within the team and support the team lead with Datix investigations, and development audits. You will deputise for the Band 7 Occupational Therapist in their absence.

Overview of Clinical Responsibilities

To be professionally and legally accountable for all work undertaken. To work within the standards and guidelines of the team, the Trust, and professional organisations.. To ensure that the client and/or their carer has an understanding of treatment protocols and gives informed consent to examination and treatment. To successfully manage, modify and prioritise a caseload of clients with complex needs, with supervision and guidance as necessary from senior staff, taking a key worker role as indicated. To undertake a range of neurological and/or complex multipathology occupational therapy assessments of clients with complex needs through subjective questioning, objective examination and the use of other resources. To prioritise factors involved in the clients presenting condition through clinical reasoning based on knowledge of current professional opinion, clients prognosis and other physical, social, emotional and mental health issues. To agree personalised goals for treatment with clients and/or their carer and ensure that they are supported through their rehabilitation key worker, carer assessment and specialist sign posting routinely occurring. To maintain a high standard of clinical practice which is evidence-based, and uses standardised assessment and outcome measures, where appropriate. To deliver evidence based treatment programmes within a team approach in a variety of settings and evaluate and review occupational therapy treatment/interventions within a multi-disciplinary framework and modify as necessary. To deliver routine and specialist Occupational Therapy interventions with clients assessing their occupational needs and addressing quality of life, and physical, cognitive, affective, social, and environmental issues utilising appropriate individual or group treatment modalities. To use a range of interventions within rehabilitation and complex case management when managing tone, motor recovery, increasing participation in ADLs. To actively identify the need for joint working opportunities To delegate and monitor appropriately work to rehabilitation assistants To work autonomously at times in isolated community situations which vary in relation to local socio-economic and multi-cultural needs. To use knowledge of other multi-disciplinary roles and external agencies to ensure effective referral management and achieve best client care including seamless transfer of care. To have the ability, knowledge and skills required for moving and handling people in hospital and community environments and supporting the formulation of a routine risk assessments/ handovers. To be physically able to install specialist occupational therapy equipment in client homes applying safe manual handling at all times, especially when doing so in restricted spaces To advise and teach the relevant carers/health workers to participate in on-going rehabilitation care programmes for clients, as appropriate and ensure that self-management activities are embedded routinely into rehabilitation and transfer of care plans . To participate in routine multidisciplinary assessments of need across health and social care prior to transfer of care ensuring comprehensive risk assessment and specialist rehabilitation prescriptions are used to support with appropriate and timely discharge preparation and referral onwards as needed To actively be involved in the development of new specialist clinical services within the Team. To be able to manage clinical risk in own caseload and support other staff to comply manual handling risk assessment and local policy regarding lone working.
