Job summary
We provide specialist diabetes care to the 21,000 people living with diabetes across North East Essex, in the form of specialist diabetes clinics to the most complex cases, young persons, pre pregnancy, antenatal, insulin pump and MDT foot clinics, numerous diabetes patient structured education courses and a strong emphasis on up skilling and supporting primary care via education sessions, mentorship and in practice support alongside supporting delivery of Diabetes Masters modules at the University of Essex.
We pride ourselves on our close collaborative working with multiple local community services, lifestyle services, health in mind, pharmacy and the CCG. We are an enthusiastic, friendly team and strive to provide a patient centred, safe, effective and high quality service. Regular CPD is scheduled for the team and on an individual basis to support ongoing evidenced based practice and encourage audit and research within the service.
Main duties of the job
The Diabetes Specialist Practitioner encompasses professionals from a range of disciplines including Registered Nurses, Registered Dietitians or Allied Health Professions.
The advanced role is characterised by high levels of clinical skill, competence and autonomous decision-making abilities within their own discipline whilst specialising in diabetes.
The post holder will work as an autonomous practitioner in the provision of specialist clinics providing highly specialist, evidence based and high-quality diabetes care supporting people with diabetes across North East Essex as defined by the service needs.
The role will also involve working with our Primary Care colleagues to deliver a holistic approach to diabetes management according to the practice requirements. This may include direct patient contact, mentorship for developing staff and audit using population health management proactive care to enhance the practice outcome

Diabetes Specialist Nurse
Main Duties & Responsibilities
To act as an advanced autonomous practitioner in their area of specialism who is professionally and legally accountable and responsible for all aspects of work including decisions about treatment and management of complex patients.
To maintain accurate and comprehensive documentation.
To hold a clinical caseload of those with complex needs and requiring advanced intervention, within a patient population using a high level of dexterity and accuracy in providing technical and clinical interventions aimed at improving services and patient outcomes.
To where appropriate, prescribe appropriate treatment as an Independent non-medical prescriber To act as a specialist Practitioner recommend treatment regimens to optimise clinical outcomes in accordance with local policies.
To demonstrate advanced clinical reasoning and decision making and the ability to analyse complex facts or situations which may be conflicting and requiring appraisal of a range of options
