Suffolk GP Federation CIC is seeking a highly skilled and experienced Diabetes Specialist Nurse to join our team. As an advanced practitioner, you will be responsible for providing specialist diabetes care to patients across North East Essex.
Key Responsibilities- Act as an autonomous practitioner in the provision of specialist clinics, providing highly specialist, evidence-based, and high-quality diabetes care.
- Work collaboratively with Primary Care colleagues to deliver a holistic approach to diabetes management.
- Maintain accurate and comprehensive documentation of patient care.
- Hold a clinical caseload of patients with complex needs and require advanced intervention.
- Prescribe appropriate treatment as an Independent non-medical prescriber.
- Recommend treatment regimens to optimize clinical outcomes in accordance with local policies.
- Dietetic or health-related degree or equivalent qualification.
- Current HCPC registration.
- Post-registration training in Diabetes.
- Clinical supervisory skills course.
- Training on CBT or motivational interviewing.
- Non-medical prescribing qualification.
- Insulin pump training qualification or equivalent experience.
- MSc in Diabetes or related subject, or willingness to work towards.
