Main duties of the job

We are currently looking to recruit a dynamic, enthusiastic and flexible individual that will perform a pivotal role within the Early Intervention Community Team, giving you the opportunity to show how you can provide an innovative service to adults within the community as well as contributing to a comprehensive care plan for the individual service user.

The Early Intervention Community Team is focused on preventing avoidable hospital admission and facilitating discharge. You will have the opportunity to work alongside Nurses as well as Therapists in meeting these goals.

Qualifications

Essential

NVQ Level 2 in Health & Social Care English and Maths GCSE/Good standard of education

Experience

Essential

Relevant experience in delivering direct patient care in a health or social care setting.

Desirable

Experience of working in a rehabilitation setting.

Skills/Knowledge

Essential

Effective oral and written communication skills Basic IT Skills Able to work in a team Able to work independently Awareness of the needs of a multicultural, socially diverse population. Awareness of responsibilities related to Safeguarding Vulnerable adults and children
