The Team is responsible for assessment and treatment to prevent inappropriate hospital admission, rehabilitation to patients with chronic and long term conditions, end of life Palliative care to patients in their own homes and also rehabilitation to patients in care homes. There are numerous opportunities to work alongside other professionals within the integrated community services and to be part of a dynamic team of professionals.

It is essential that you have experience of rehabilitation or a caring role, preferably through employment within a community setting but we would consider applicants with transferrable life skills from other caring roles. Applicants will need to have good verbal, written and team working skills. You will need to be flexible in your approach and confident to work alone. Basic IT skills andNVQ level 2 would be essential.
In return for your enthusiasm and commitment, we can provide an opportunity for you to develop your clinical skills and experience in a supportive team, with regular supervision and appraisals. We are proud of our commitment to the training and development of staff and may offer excellent opportunities for development through the Apprenticeship scheme. This is a permanent post.
