Job summary

An exciting opportunity has arisen for an experienced, forwardthinking and enthusiastic clinical pharmacist to join our CDDFT Orthopaedic team.The post holder will make significant contributions within themultidisciplinary teams to enhance peri-operative care to patients admitted toboth emergency and elective wards.

You will be based predominantly in surgery at DarlingtonMemorial Hospital where there is an established pharmacy service. Attendance atother sites, including the University Hospital of North Durham and BishopAuckland General Hospital may be required in order to deliver key objectives.

Main duties of the job

Should you be successful, you will work closely with the consultantsand wider MDT to deliver the trust values of safe, compassionate and joined upcare. Our mission is 'right first time, every time'.

You will demonstrate leadership in the orthopaedicspeciality and provide specialist advice on medicine use. This will includeprofessional collaboration to safe peri-operative care and to support the teamwith evidence-based practice and pharmaceutical issues.

You will build on well established relationships betweenpharmacy and the orthopaedics team and a key part of the role will be todeliver the governance agenda, teaching, quality improvement, servicedevelopment and sustainability. We willsupport the successful candidate with mentoring and development opportunitiesincluding practicing as an independent prescriber, advanced/ consultant portfoliodevelopment and other further education opportunities.

You will provide leadership to other pharmacystaff and will support the Lead Pharmacists in moving the trust's pharmacystrategy forward. You need to be able to work autonomously, whilst motivatingothers and being a team player.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Job responsibilities

Thepost holder is responsible to and managed by the Lead Pharmacist and isprofessionally accountable to the Chief Pharmacist / Clinical Director.

Thepostholder manages a team of clinical pharmacists, pharmacy technicians andpharmacy support workers and is responsible for ensuring the planning and deliveryof safe, effective and efficient clinical pharmacy and medicine managementservices to an agreed standard to a designated group of patients and serviceusers.

Thepostholder delivers specialist clinical pharmacy and medicine managementservices, represents the directorate at speciality meetings and provides specialist advice and expertise to allmembers of the directorate, the Trust and the wider health economy.

They work closely with patients and healthcareprofessionals including consultant staff and Care Group leads to ensureservices are developed and delivered in a manner that is consistent with thepriorities and needs of patients and other service providers.

They liaise with otherpharmacy teams, healthcare professionals and agencies both within and externalto the Trust to consider medicine management issues across boundaries

They influence local practice toensure safe medication use, and to ensure that their own practice and thepractice of others effectively acknowledge good prescribing practice, patientsafety and risk minimisation.

They are a member of the site based seniorpharmacy management team, contributing to strategy, policy development,operational and financial planning

Essential

Registered as a Practising Pharmacist with the GPhC

Desirable

Membership of RPS or a relevant Clinical Pharmacy Organisation appropriate to the subject area

Special Skills & Knowledge

Essential

Good understanding of medicines reconciliation and how it fits into the larger function of hospital pharmacy. Ability to apply theoretical knowledge to ward/clinic based situation. Provision of complex information on medicines use, clinical pharmacy and financial issues to senior Consultants and managers. Membership of regional / national / international specialist interest groups relevant to field of practice. Demonstrable leadership, management and supervisory skills. Demonstrable teaching / tutoring skills with evidence of ability to deliver training to a wide-range of healthcare staff, pharmacy, nursing, medical. Involvement in development of evidence-based clinical guidelines and protocols. Sound knowledge of MI resources. Good understanding of current pharmacy issues. Demonstrable understanding of key drivers, financial pressures and constraints within the NHS.

Desirable

Ability to demonstrate an area of expertise within a field of pharmacy practice. Evidence of active contribution to relevant specialist interest groups. Record of research and/or audit and/or scholarly publications

Qualifications

Essential

Masters Degree in Pharmacy or equivalent Completed a suitable postgraduate qualification (eg Clinical diploma) or demonstrated sufficient experience to meet RPS post-registration foundation competence standards Evidence of CPD and education Independent Prescriber or willing to work towards

Desirable

Post graduate Masters qualification in a relevant pharmacy subject. Post graduate management qualification. Working towards a development portfolio, credentialing at advanced practice defined by the RPS. Teaching experience in clinical and classroom settings

Experience

Essential

Experience in leading and developing highly developed near-patient clinical pharmacy services. Able to work as an autonomous practitioner in all areas of pharmacy practice. Experience of developing good relationships with other health care professionals. Experience of providing directorate pharmacist services budgetary reports, drug usage reviews, development of drug protocols, specialist drug information etc. Experience of both undertaking and supervising audit of clinical practice. Evidence of involvement in multidisciplinary service development
