There is an exciting opportunity at the Royal Marsden, we are currently looking for a dedicated, motivated Speech and Language Therapist to join the Senior Adult Oncology Programme on a fixed term contract. A multidisciplinary approach to the older adult with cancer is a gold standard for the management of these patients. The Senior Adult Oncology Programme (SAOP) was developed at The Royal Marsden to personalise their care and improve outcomes, following their cancer treatment. This is a wonderful opportunity to work closely with Dietetic, Physiotherapy and Occupational Therapy colleagues providing personalised care to this group of patients. 

Main duties of the job

To take the SLT lead on tracheostomy management within the Trust including leading on the development of protocols and programmes in this clinical area, working closely with and actively participating in MDTs; liaising with other specialist hospitals to ensure appropriate common care pathways are established and effective and appropriate information is maintained. To take the SLT role in the development and delivery of teaching and training programmes for professionals and other staff as required. To be responsible for disseminating knowledge, within the Trust and on a national and international basis, through reading, participation in research and advanced clinical practice.

Detailed job description and main responsibilities

To demonstrate highly specialist clinical skills for interpretative swallowing assessment techniques and interventions involving instrumental procedures e.g. nasendoscopy (FEES) and videofluoroscopy (VDF). To arrange, implement, analyse and write reports on findings and to report findings to relevant MDT with management recommendations and to plan specialised therapeutic programmes of care. To become an IRMER referrer, following an IRMER course and internal RMH training and then requesting video fluoroscopies in conjunction with the IRMER operator. To use and demonstrate specialist knowledge to inform sound clinical judgements/decision making for complex case management and advise others on the same, often in the absence of robust evidence base. To make appropriate differential diagnoses and highly specialist clinical decisions based on assessment findings of complex cases.

Essential criteria

Registered MRCSLT Registered member of HCPC Masters or equivalent of post-graduate specialist training and membership/committee role in relevant CEN Attended post graduate dysphagia training leading to independent practice. Attended specialist post graduate study relevant to ENT or Head & Neck cancer e.g advanced dysphagia course including tracheostomy management and Surgical Voice Restoration. Attended specialist/expert short courses relevant to head & neck cancer e.g advanced videofluoroscopy and/ or FEES Post graduate training/experience in videofluoroscopy

Desirable criteria

Advanced counselling Post graduate training/ experience in videofluoroscopy with patients with laryngectomy Post graduate training in the use of AAC with adults.

Demonstrable experience of working at a specialist level with adults with acquired dysphagia/ communication disorders as a result of their cancer and /or its treatment, particularly Head & Neck or ENT. Demonstrable experience of independent management of adults with acquired dysphagia particularly, tracheostomy and laryngectomy/SVR Knowledge of artificial larynx/oesophageal voice Evidence of developing and delivering training Clinical supervision of SLT students Experience of developing case management skills in less experienced staff Knowledge/experience with the use of nasendoscopy/stroboscopy when assessing/treating patients with swallowing difficulties. Demonstrable experience of an ability to independently perform and evaluate videofluoroscopy assessments at an advanced level.
