An exciting opportunity has arisen for a Band 7 Outreach Team Leader to join the Specialist Autism Team. We are a unique and continuously developing service, working closely with secondary mental health care services to prevent unnecessary hospital admissions for autistic individuals.
This post is a fantastic opportunity for a creative and innovative practitioner with a passion for delivering person-centred care, using a team formulation approach in conjunction with care planning with the patient. We are looking for an exceptional Band 7 practitioner who is able to demonstrate daily clinical leadership to the staff team to ensure the delivery of high quality specialist, bespoke services which are designed to meet individual need and preference.
The Specialist Autism Team adopts a multidisciplinary team approach to working which supports the evolutionary scope of the service. The MDT is made up of Occupational Therapists, Social Workers, Nurses (RMN & RLD), Speech and Language Therapists, Therapy Assistants & Clinical Psychologists. Through such an approach we are continually learning, developing and findings different ways of working in order to meet the needs and wishes of the individuals we work with. For this reason we have regular team development sessions and each team member plays a vital role in sharing knowledge and information as we learn together as a team.
Main duties of the job
As part of the leadership team, the post holder will be responsible for providing expert support, advice clinical and operational leadership to the Specialist Autism Team to offer an enhanced model of support to community, acute and specialist mental health inpatient settings, the wider mainstream mental health and social care services within Lancashire and South Cumbria.
As Team Leader the post holder will support the operational aspects of service delivery and report directly to the Service Manager. These include helping to manage budgets, recruitment, staff management (rosters, sickness, leave etc), developing Standard Operating Procedures, managing risk and other operational tasks.
The role includes a clinical & professional component also, the Team Leader will clinically support the staff in the team providing supervision and guidance as appropriate and carrying a reduced caseload to reflect the joint Operational and Clinical nature of the role.
The Specialist Autism Team (SAT) operates a continuous care pathway across three key domains: Outreach, which focuses on admission prevention. Inreach, which focuses on working with inpatient settings to improve the efficacy of hospital admissions and shorten length of stay and Discharge Coordination, which seeks to ensure that discharge planning begins early, is thorough and robust and supported by the whole health and care system, contributing to shorter length of stay and reduced readmissions.
