Job summary

North Uttlesford PCN has a Maternity leave cover opportunity for a senior care coordinator to join our primary care network (PCN), to support our 4 GP practices across the North Uttlesford area to assist in re-shaping primary care across our communities.

The role involves working very closely with the practices and the multidisciplinary team (MDT) within the PCN, and is pivotal in ensuring that all patients receive the best possible care and service.

If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you.

Main duties of the job

supporting the 4 local GP Practices in coordinating all key activity, including access to services, advice, and information, as well as ensuring that health and care planning is timely, efficient, and patient-centred. supporting digital initiatives. responsibility for the co-ordination of the patients' journeys through primary care services.

About us

Our primary care network (PCN) is based on 4 north Uttlesford practices working together. PCNs build on existing primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for people close to home. Clinicians describe this as a change from reactively providing appointments to proactively caring for the people and communities they serve.

We are a growing, supportive team of staff led by our clinical director Dr Richard Boyce, GP and nurse educators, 4 pharmacists, paramedics, care-coordinators, health and wellbeing coaches, an integrated mental health team and social prescribers.

Person Specification

Qualifications

Essential

Ideally you will have a Diploma Level 2 in Health and Social Care, or an equivalent qualification.

Experience

Essential

Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups; Strong listening and communication skills; Familiar with local resources and services, including how to access them; Motivated by helping people, with care and empathy; Able to deliver person-centred support in a non-judgmental way; Experience of completing holistic person-centred care planning assessments. Primary Care experience in a senior admin or management role. Experience of using SystmOne and EMIS clinical systems. Experience in working with patients and as part of a wider MDT
