Urology Cancer Triage Nurse

Job summary
To triage all Urgent Suspected Cancer GP referrals into the Urology service and conduct telephone assessments or face to face assessments as appropriate to determine further investigations required in collaboration with the Consultant team.
To ensure that patients requiring a Consultant outpatient appointment are seen
Work closely with the Urology Early Diagnosis Support Worker to minimize delays and ensure that all suspected urology cancer patients experience the same level of care and have the same chance of receiving a definitive diagnosis or exclusion of a urology cancer in line with the 28 day Faster Diagnosis Standard.
Main duties of the job
Provide specialist, expert clinical knowledge and advice to clinical colleagues, patients and carers / relatives.
Evaluate service delivery, identify areas of improvement and initiate change.
Ensure the effective and efficient use of physical and financial resources, make recommendations regarding supplies and equipment.
Effectively manage time and caseload in order to address the needs of a changing and developing service, planning and organising a broad range of activities.
Monitor health, safety and security of self and others and promote best practice in the clinical area.
To co-ordinate and review the urology service to ensure a safe a cost effective service.
To produce and implement policies and guidance in line with evidence based practice in urology
Identify potential areas for service improvement.
Participate in the production of reports of progress and report to relevant Trust committees.
Provide representation at meetings where appropriate.

Job responsibilities
Perform comprehensive assessment of patient needs, plan, implement, co-ordinate and evaluate care delivery according to diagnosis.
Perform skills in assessment and / or diagnosis and/or treatment.
Collect, collate, evaluate and report information to the MDT, maintaining accurate patient records.
Involve patients and carers / relatives in the planning and delivery of care and development of services.
Work collaboratively with other professionals and services to ensure patient needs are met, especially in relation to ongoing care needs.
Establish and maintain effective communication with patients and professionals across health and social services, providing and receiving complex and sensitive information.
To provide specialist advice to urology patients, relatives and other multidisciplinary team members.
To act as a specialist resource for the management of urology patients in primary and secondary care settings
Be an initial point of contact for Primary Care queries.
Recognise and respect the cultural and spiritual needs of others, as well as ensuring that the service is fully accessible to those with additional needs or requirements after considering equality impact assessments.
Communication of highly sensitive information and knowledgeably discuss all options along the pathway. Where appropriate, act as advocate to enable the patient to undertake informed decision making.
