Job summary

The Nurse Practitioner will act within their professionalboundaries to provide patient care from initial triage, history taking,clinical assessment, diagnosis, treatment and evaluation of care in the surgeryand on home visits. The NursePractitioner will demonstrate critical thinking in the clinical decision makingprocess. They willwork flexibly and supportively as a key member of the nursing team and widermultidisciplinary practice team maximising efficiency and skill mix.

Therole is to complement our existing team of excellent Practice Nurses and HCA’swho undertake all basic nursing duties, health promotion, chronic diseasemanagement and minor illness assessments. You will be expected to work autonomously and undertake face to face clinicalassessment of your patient list in 15 minute appointments, making clinicaldecisions based on experience, knowledge and analytical ability of undiagnosedpresentations. Experience of SystmOne clinical software would be an advantage

The applicant must bea Nurse prescriber, you must be willing to undertake home visits, makeappropriate clinical referrals and process pathology results and hospitalcorrespondence for your patients.

Registered General Nurse Nurse Practitioner Degree Non-Medical Independent Prescriber

Experience

Several years post registration Experience in managing long term conditions Experience in minor illness, clinical decisions, telephone triage Experience of working autonomously

Knowledge of the working of general practice Up to date knowledge of health agenda, local and national Management of diseases, undiagnosed health roles Working autonomously Excellent interpersonal and communications skills Ability to work effectively in a multidisciplinary team as a key player Skills in clinical examination and diagnostic techniques. Knowledge of PGD & current prescribing regimes Awareness of the legal and ethical issues of the role Information Technology Skills. Knowledge of PanMersey Guidelines.
