Job summary

BOC have an openingwithin the combined Home Oxygen and Pulmonary rehabilitation service in North Lincs . We arelooking for an Exercise physiologist with experience in delivery / supporting Pulmonary rehabilitation. The role includes;

Undertaking comprehensive subjective and objective assessments ofrespiratory patients on the pulmonary rehabilitation (PR) pathway.

Leading the delivery of group, remote and virtual PR sessions aligned to PRSAS standards.

Prescribing individualised exercise programme with continual assessment and progression and onward lifestyle changes discussions.

Participate in NRAP audit and support the workstreams to achieve full PRSAS accreditation

Clinical support to PR technical instructors team

Communicating effectively and working collaboratively with colleagues toensure delivery of a coordinated service

Receiving ongoing clinical supervision with a qualified mentor with agreater respiratory knowledge base

Working with Nursing team identifying patients who may require ambulatory or long term oxygen therapy at home.

Main duties of the job

The HOS-AR and PR team work closely together managing the work load and prioritising the most urgent cases. Your role will be based in PR with face to face delivery from the PODS leisure center twice weekly , in addition you will undertake home based telephone assessments and deliver virtual Video rehab in conjunction with British steal and local community initiatives as part of an exciting pilot funded by NHSE.

The service runs Monday to Friday ( Exc BH & weekends)

You will hold a degree in exercise physiology ( registered or working towards professional AHCP registration ) or adegree inPhysiotherapy with HSPC registration

Ideally, you will have 1 yrs post-registration experience in Pulmonary rehab delivery

Have a keen interest in Respiratory medicine

Be able to take and document clinical observations and scoring tools( PHQ9 ,CAT score)

Excellent understanding of Respiratory anatomy and function

Understanding of more common pathophysiology in respiratory

Competent with IT and EPR

Hold a Full clean driving license

Our North Lincs service has the advantage of also providing the Home oxygen assessment service, offering the chance to further your knowledge and skills in ambulatory oxygen assessment and treatment if desired.

To have comprehensive understanding of patient presentation based upon high standards of clinical knowledge and to assess suitability for pulmonary rehabilitation based on knowledge and experience. To undertake a comprehensive subjective and objective assessment of respiratory patients and identify when further clinical support is required for those patients with diverse or complex presentation or multiple pathologies. To prioritise patient problems from this assessment and formulate an agreed treatment programme. Use clinical reasoning skills to accurately identify impairments and disabilities. To formulate, plan, deliver, evaluate and progress a patient-centered pulmonary rehabilitation programme by effective clinical reasoning, using current evidence based practice. To recognise when rehabilitation is not the most appropriate course of action. To use valid and reliable outcome measures in line with evidence based practice and best practice statements to evaluate patient progress, reassessing and adjusting treatment programmes as indicated. Communicate effectively and work collaboratively with colleagues to ensure delivery of a co-ordinated service. To be flexible so to facilitate effective verbal and non-verbal communication with those demonstrating communication barriers. Use advocates and other communication strategies where necessary. Work with the Team Leader and Regional Clinical Lead in the evaluation of the Pulmonary Rehabilitation service using evidence based practise projects, audits, outcome measures and research. Assist with research led by other members of the team. Provide patient related data/statistics when requested in a timely manner. Organise own caseload effectively and efficiently with regard to prioritisation and use of time. To recognise when patient presentation requires referral onto other health professional or specialist teams, and take appropriate action. Person Specification

Essential

ABG sampling and interpretation of results Conducting walk tests, ensuring the patient receives the most appropriate ambulatory provision Confident in using computer and electronic patient management software Highly motivated and capable of self managing

Desirable

Experience in pulmonary rehabilitation Experience analysing arterial blood gases Asthma / COPD diploma ARTP Certification in ABG Sampling or willingness to undertake
