Job overview

To work within a multi-agency team bridging hospital and community based services to deliver high quality care to all COPD. To coordinate and deliver high quality patient care as detailed in the BTS COPD Care bundle to facilitate admission avoidance and ensure safe, timely discharge of patients.

To be responsible for completion of a discharge bundle for all patients admitted with an exacerbation of COPD in order to optimise ongoing, personalised care and reduce risk of re-admission. This will require close working with hospital and community multidisciplinary and multi-agency teams.

To maintain patient related documentation according to UHSussex NHS Trust and NMC standards, Code of Conduct, and to use Council and Sussex Community Trust documentation and IT systems where appropriate.

As a specialist practitioner in COPD to provide leadership, to teach, supervise and appraise less experienced MDT staff to ensure a high quality of clinical care and adherence to the delivery of the discharge care bundle.

To adhere to Trust policies and Professional bodies Code of Conduct and be guided by National Standards where they exist. To contribute to the achievement of CQUINS and Quality Standards set by commissioners for COPD patients.

Main duties of the job

To be responsible and accountable for the delivery of patient care and to influence practice Trust-wide for COPD. To be professionally and legally responsible and accountable as an autonomous practitioner for all aspects of own work. 

To be highly competent in assessment, treatment and management of respiratory patients, including those with highly complex medical and social presentations using advanced theoretical knowledge of anatomy, physiology and pathology and to be responsible for effective discharge planning, often in situations where conflicting evidence is present.

To perform and interpret spirometry and capillary blood gases and to recommend/ prescribe (if prescriber) appropriate dosages of medication and oxygen in line with evidence based guidelines.

To interpret and analyse clinical and non-clinical facts which may be conflicting using underpinning theoretical knowledge and experience to provide/contribute to diagnosis, identification of main problems and anticipated prognosis working closely with inpatient medical and community respiratory teams.

Detailed job description and main responsibilities

 Plan and prioritise own time to ensure patient care, training, supervision, a wide variety of meetings (including management, service development a with other health professionals) are undertaken. To assist less experienced staff in this undertaking. To use excellent prioritising and time management skills to meet the unpredictable and conflicting demands of the service covering ED, wards and community patients. This will require liaison with the respiratory medical team and Community Respiratory team. To be responsible for identifying and implementing service development and to ensure effectiveness of patient care is continually monitored and improved. To ensure that the service for complex lung disease responds to national initiatives and policy within the framework of clinical governance including clinical effectiveness, evidence-based health care, managing clinical risk, and research and development To motivate, explain, educate and encourage patients and their carers to undertake treatment and adhere to education and long-term management programme to maximise rehabilitation potential, enable effective discharge and avoid/minimise readmission.   To assess capacity to give consent, gain informed consent and have the ability to work within a legal framework with patients who lack capacity to consent to treatment such as patients who have dementia, learning difficulties or who are acutely confused. To demonstrate expert ability to clearly communicate complex and sensitive information to staff, patients and carers. This may include explaining diagnoses or prognoses about long-term/permanent disabilities and loss of mobility and independence where an educational and counselling role is required to be highly developed. To adapt communication with individuals who have a range of communication difficulties, including dementia, dysphasia, deafness and individuals for whom English is a second language and ensure all members of the team do likewise. To be able to diffuse potentially hostile and antagonistic situations with staff, patients and relatives using highly developed negotiation and interpersonal skills. To maintain accurate, comprehensive and up-to-date documentation in line with legal requirements and communicate assessment and treatment results to the appropriate disciplines in the form of reports and letters. To maintain close links and work collaboratively with all staff and people involved in patient care, student education, research or policy development and always promote good working relationships. To represent the respiratory nursing/medical department on specialty issues within the Trust and external agencies.  Ensure timely and effective communication with all MDT staff on professional matters and cascade Trust/corporate information to staff. To develop specialty standards based on Care Bundle and other national COPD standards where they exist and on best evidence. To ensure that these standards are monitored and adhered to by the MDT. To plan and undertake evaluation of practice and performance through clinical audit and studies, formulate reports and make recommendations for change. To support less experienced staff within the MDT service to undertake clinical audit. To demonstrate a sound understanding of Clinical Governance and Risk Management and ensure its effective application to the work situation. To resolve verbal complaints and report all formal complaints, incidents and accidents to service leads as soon as possible, document according to Trust policy and carry out preliminary investigation. Act as an expert practitioner demonstrating specialist knowledge, skills and expertise within the area of work and within the context of changing health care provision. Put quality at the heart of practice by delivering evidence based individualised and personalised care through holistic needs assessment, planning and evaluation of care for patients. Ensure that the quality of patient care is assured by involvement in the implementation of regular benchmarking of the standards.  Act as a role model and ensure effective staff supervision with clinical practice, work with staff and patients in an advisory, teaching and support capacity and maintain professional competence, and credibility. Ensure privacy and dignity of patients and clients is key in provision of nursing care and clinical practise of all staff. Ensure any safeguarding issues are raised appropriately to lead nurses to enable investigation in relation to vulnerable adults or children.
