Home First

For most people the best place to recover after being in hospital is at home. Once hospital treatment has finished and a patient is on the mend, the Home First team will help the patient to get home as soon as possible and will ensure they have the support they need at home including care, physiotherapy and/or occupational therapy. Our aim is to support the patient recovery at home and help them maintain their independence.

Main duties of the job

What you can add:

You will be professionally and legally responsible for all aspects of your own work as an autonomous practitioner, including the management of patients in your care.

You will help with patients that may need to re-register with their GPs.

Helping the patients families to help the decisions that have been made / reported.

You will support case coordinations linking / attending Multi Disciplinary Team meetings,

Youll support and manage the Continuing Healthcare Checklist (CHC) paperwork - fast track and make sure there are adequate referrals.

Excellent communication skills

Ability to work on some initiative and desire to develop the role

Job description

Job responsibilities

Youllmanage in recognising deteriorating patients

Youll working with the GPs of the care homes that are notknown to the patient already.

Youllsupport and manage the Continuing Healthcare Checklist (CHC) paperwork - fasttrack and make sure there are adequate referrals.

You willsupport case coordinations linking / attending Multi Disciplinary Team meetings,

You may link in with diabetes nurses Developing relationships with all the services.

This is a timed service quick responses to the needs ofthe patient.

Helpingthe patients families to help the decisions that have been made / reported.

Youwill help with patients that may need to re-register with their GPs.

You will have regular check ins making sure they have completedtheir actions joined the GPs

Youwill review and support the actions behind the discharge summary of patients.

You will be required to work from the care homes, asdirected working flexibly.

Certain care homes have their own meetings specific times/ days some working from home flexibility depending on the need to seenew/existing patients.

**If working from home you would be expected to be incounty.

Know how to link in with social work discharge planningprocess (Home First county)

Qualifications

Essential

HCPC OR NMC registration Occupational Therapist or Physiotherapist equivalent Qualification OR Nursing Qualification

Experience

Essential

Evidence of experience with Complex Cases and Discharge Planning Understanding of both NHS Health and Social Models Experience of Audit and Data collection Good with Excel

Desirable

Desirable - Ideal if knowledge of linked services ( Age UK)
