Job summary

Thisis an exciting new opportunity; therole will be right at the heart of shaping Naomi House and Jacksplace for thefuture. Working closely with the Director of Care (as Registered Manager), the successfulAdvanced Nurse Practitioner (ANP) will be the first point of contact forchildren and young adults with all types of health needs in-house, working asan autonomous practitioner and in conjunction with other professionals to makedecisions and apply appropriate nursing interventions to the children and youngadults. This will include history taking, clinical assessment,diagnosis, treatment, and evaluation of care; working within the Hospices andNMC policies, protocols, and guidelines, demonstrating safe clinical decisionmaking, high levels of critical thinking and holistic practice. You will beexpected to act independently but will have access to a Consultant/AssociateSpeciality Doctor for advice and additional support as necessary.

Main duties of the job

You will also be expected to be a senior clinicalleader and expert within Naomi House and Jacksplace, and at times workalongside other care teams and professionals within hospital settings and thecommunity.

As aqualified Advanced Nurse Practitioner, with experience working with childrenand young adults you must be able to demonstrate safe clinical decision-making, andsound clinical expertise and knowledge when working with children and young adults.The post holder will support the leadership team to meet the needs of theservice users and contribute to service improvements within an environment undergoingchange and service development. You will need drive and determination,excellent interpersonal and communication skills and a passion and vision that motivatesothers.

JobSummary

TheAdvanced Nurse Practitioner (ANP) will be the first point of contact for childrenand/or young adults with all types of health needs in-house. Key Responsibilities

Practiceas an ANP within own sphere of competence and agreed acceptable limits ofpractice being accountable for own professional actions.

Beresponsible for assessing, diagnosing, interpreting investigations, andtreating the children and young adults within the In-House Hospice servicewithin a developing scope of competence.

Towork as part of the multi-disciplinary team in providing quality evidence-basedhealth care to meet the immediate needs of the children and young adults.

Tobe responsible for contributing to the assessment of care needs and thedevelopment, implementation, and evaluation of programs of care.

Toteach other clinical and non-clinical staff, supporting competency assessmentsand sign off.

Tosupport day-to-day clinical leadership to the clinical team, participatingfully in the delivery of healthcare provision.

Toutilise clinical expertise and experiences towards developing and enhancingservices within the Hospice and the wider health and social care economies

Demonstratethe development and implementation of advanced skills and competencies andensure that evidence-based practice is inherent in all aspects of care andtreatment.

Ensurefull and accurate documentation in the children and young adults notes ofclinical history, physical examination, diagnosis and treatment plan.

Co-ordinate,and participate as required in any research projects and clinical audits.

Amember of the Clinical On-Call rota, providing out of hours support for thehospice teams.

ManagerialResponsibilities

To ensure all resources,human, financial and environmental, are used in the most economic,efficient and equitable way.

To ensure that Health and Safety requirementsare met and that risks to service users and staff are assessed and action takento minimise these.

To participate in the development, monitoring and audit of policies and SOPs relating to clinical practice andcare,and ensure that these are adhered to acrossall areas of work with children, young people and families.

To act as a representative of the organisation takingpart in professional relationships, collaborative working and externalpromotional activities as appropriate encouraging partnership working.

ClinicalResponsibilities

Assess,diagnose, plan, implement and evaluate treatment/interventions and care for thechildren and young adults presenting with undifferentiated and undiagnosedconditions and complex needs.

Clinically examine and assess patient needs from aphysiological and psychosocial perspective, and plan clinical care accordingly.

Proactively identify, diagnose and manage treatment plans forthe childrenand young adults at risk of developing a long-term condition (asappropriate).

Diagnose and manage both acute and chronic conditions,integrating both drug and non-drug-based treatment methods into a managementplan in liaison with the child, young adult and family as appropriate.

Ensure that where a child or young adultrequires referral to a service for their health needs it is made by the same day.

To refer appropriately childrenand young adults to other healthcare professionals as required.

Prescribe and review medication for therapeuticeffectiveness, appropriate to the children and young adultsneeds and in accordance with evidence-based practice and national/localprotocols and within scope of practice.

Provide information and advice on prescribed orover-the-counter medication on medication regimens, side-effects andinteractions.

Prioritise health problems and intervene appropriately toassist thechildren and young adults in complex, urgent or emergency situations, includinginitiation of effective emergency care.

Always working within your scope of professional practice, recognising your limitations and when to seekadvice and support from peers. To ensure you provide the children andyoung adults with robust safety netting advice in relation to theirhealth need providing a safe transfer.

Work supportively alongside clinical and administrativecolleagues in ensuring the highest standards of care for all the children andyoung adults.

Maintain accurate and contemporaneous records.

Experience of working as an ANP in a Ward environment Relevant experience of caring for children or young people or both with highly complex care needs and disability. Experience of holistic assessment and individualised planning of care. Experience of effective collaborative working with professionals from other agencies and disciplines. Experience of working with children and/or young people with palliative care needs. Experience of clinical audit. Experience and good understanding of clinical supervision and reflective practice. Knowledge and experience of various physical and mental health conditions associated with children, young people with life limited diagnosis and palliative care.
