Advanced Nurse Practitioner

This is an exciting opportunity to join our team as an Advanced Nurse Practitioner, working closely with the Director of Care to provide high-quality palliative care services to children and young adults. As a senior clinical leader, you will be responsible for assessing, diagnosing, and treating patients with complex needs, working autonomously and in conjunction with other professionals to make decisions and apply appropriate nursing interventions.
Main DutiesKey responsibilities include:
- Assessing and diagnosing patients with complex needs, including history taking, clinical assessment, and diagnosis
- Developing and implementing individualized care plans, working within Hospices and NMC policies, protocols, and guidelines
- Providing high-quality palliative care services, including symptom management, respiratory support, and nutritional care
- Working collaboratively with other professionals, including doctors, therapists, and social workers, to provide comprehensive care
- Supporting the leadership team to meet the needs of service users and contribute to service improvements
