Advanced Nurse Practitioner

This is an exciting opportunity to join our team as an Advanced Nurse Practitioner, working closely with the Director of Care to provide high-quality care to children and young adults with life-limiting conditions.
Main ResponsibilitiesAs an Advanced Nurse Practitioner, you will be responsible for assessing, diagnosing, and treating children and young adults with complex care needs. You will work autonomously and in conjunction with other professionals to make decisions and apply appropriate nursing interventions.
You will also be expected to be a senior clinical leader and expert within our hospice, working alongside other care teams and professionals to ensure seamless services.
Key Responsibilities- Practice as an ANP within your sphere of competence and agreed acceptable limits of practice, being accountable for your own professional actions.
- Be responsible for assessing, diagnosing, interpreting investigations, and treating children and young adults within the In-House Hospice service, within a developing scope of competence.
- Work as part of the multi-disciplinary team in providing quality evidence-based healthcare to meet the immediate needs of children and young adults.
- Be responsible for contributing to the assessment of care needs and the development, implementation, and evaluation of programs of care.
- Teach other clinical and non-clinical staff, supporting competency assessments and sign-off.
- Support day-to-day clinical leadership to the clinical team, participating fully in the delivery of healthcare provision.
- Utilize clinical expertise and experiences towards developing and enhancing services within the Hospice and the wider health and social care economies.
- Develop and implement advanced skills and competencies, ensuring that evidence-based practice is inherent in all aspects of care and treatment.
- Ensure full and accurate documentation in the children and young adults' notes of clinical history, physical examination, diagnosis, and treatment plan.
- Coordinate and participate as required in any research projects and clinical audits.
- Be a member of the Clinical On-Call rota, providing out-of-hours support for the hospice teams.
- Demonstrate commitment to developing the ANP role within the Organisation.
- Act as a positive role model.
- Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working.
- Delegate clearly and appropriately, adopting the principles of safe practice and assessment of competence.
- Prioritize your own workload, ensuring effective time-management strategies are embedded in the team.
- Be responsible for ensuring an holistic assessment is completed for all children and young people to enable up-to-date care plans incorporating all necessary risk assessments.
- Provide support and supervision to staff and students across the wider care team.
- Work in partnership with the hospice multidisciplinary team as well as any other agencies involved in the care, to ensure the delivery of seamless services.
- Ensure all resources, human, financial, and environmental, are used in the most economic, efficient, and equitable way.
- Ensure that Health and Safety requirements are met and that risks to service users and staff are assessed and action taken to minimize these.
- Participate in the development, monitoring, and audit of policies and SOPs relating to clinical practice and care, and ensure that these are adhered to across all areas of work with children, young people, and families.
- Act as a representative of the organisation, taking part in professional relationships, collaborative working, and external promotional activities as appropriate, encouraging partnership working.
- Assess, diagnose, plan, implement, and evaluate treatment/interventions and care for children and young adults presenting with undifferentiated and undiagnosed conditions and complex needs.
- Clinically examine and assess patient needs from a physiological and psychosocial perspective, and plan clinical care accordingly.
- Proactively identify, diagnose, and manage treatment plans for children and young adults at risk of developing a long-term condition (as appropriate).
- Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan in liaison with the child, young adult, and family as appropriate.
- Ensure that where a child or young adult requires referral to a service for their health needs, it is made by the same day.
- Refer appropriately children and young adults to other healthcare professionals as required.
- Prescribe and review medication for therapeutic effectiveness, appropriate to the children and young adults' needs and in accordance with evidence-based practice and national/local protocols and within scope of practice.
- Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions.
- Prioritize health problems and intervene appropriately to assist children and young adults in complex, urgent, or emergency situations, including initiation of effective emergency care.
- Always work within your scope of professional practice, recognizing your limitations and when to seek advice and support from peers. Ensure you provide children and young adults with robust safety netting advice in relation to their health needs, providing a safe transfer.
- Work supportively alongside clinical and administrative colleagues in ensuring the highest standards of care for all children and young adults.
- Maintain accurate and contemporaneous records.
- Be aware of the safety needs of children, young adults, and families and other staff, and comply with:
- The Children Act 2004
- DoH No Secrets Guidance on developing and implementing multi-agency policies and procedures to protect vulnerable adults from abuse
- Health and Safety at Work Act 1974
- NMC codes and guidelines
- Be responsible and proactive in personal professional development by:
- Maintaining awareness of current developments in palliative care for children, young people, and families.
- Maintaining clinical skills identified in the Competency Framework.
- Ensuring timely revalidation with the NMC.
- Demonstrating commitment to the Trust's values.
