As a generic assessor you will need to be a forward thinking, autonomous practitioner, flexible, willing to develop interchangeable skills across professional boundaries, be knowledgeable on a range of conditions, their treatment and potential for deterioration. Enhanced training will be provided to assist with generic skills development.

We are inviting people with community or hospital experience to apply for this post with a professional background of nursing or therapy. This is an exciting time of forging links with our community colleagues, and embracing the current developments and projects.

Main duties of the job

Assessment

o Apply a generic model of assessment thereby streamlining the process and removing any Develop a deep knowledge of the breadth of services available to patients across health and social care.

o Analyse and evaluate highly sensitive and complex information, formulate a plan with regards to discharge planning, reaching a decision point at the earliest opportunity. Proactively seek advice from seniors in order to reach a decision Undertake any necessary training and competencies to develop and demonstrate the ability to practice as a multi skilled professional in an interdisciplinary Undertake daily functional assessments of patients on the FDRT caseload, outline rehabilitation needs to prevent the patient deconditioning whilst in the acute setting.

Communication

o Receive collate and process highly complex confidential information internal and Deploy effective negotiation skills in highly contentious situations, recognising own limitations within practice. Be able to seek guidance and support from seniors

CLINICAL LEADERSHIP

o Provide specialist advice to patients, relatives, external agencies, ward based staff, students and junior staff within FDRTo Act sensitively, confidentially and with empathy in demonstrating understanding of wide ranging physical and emotional Work in collaboration with seniors to deal with contentious sensitive and delicate issues with patients, relatives and staff.

Essential

BSc or Diploma in Physiotherapy/Occupational Therapy, HCPC Registration - Current Registered General Nurse NMC Registration - current Evidence of ongoing professional development

Desirable

Evidence of higher level degree studies. Evidence of commitment to professional development and training. An understanding of generic working and the requirements to practice as a multi skilled professional. Mentor - Nursing Students Clinical Educator - Therapy Students

Experience

Essential

Relevant experience in admission avoidance/discharge planning in either the acute/community setting. Experience in identifying risk factors and defines the management of these regarding discharging planning. Experience in the holistic assessment of a patient's needs and applying clinical reasoning to decisions. Considerable experience in performing functional assessments of patients and outlining rehabilitation plans. Considerable experience at AFC Band 5 including deputising duties, deploying staff resource to meet the demands of the service. A working knowledge of mental capacity assessment and safeguarding Therapists - Completion of core rotations Nurses - Completion of preceptorship

Special Skills/Aptitudes

Essential

Basic IT Skills The ability to analyse and evaluate highly sensitive and complex information. Recognising own limitations and escalating to a senior for a professional discussion and guidance. The ability to identify potential barriers to discharge and devise a plan to overcome these. The ability to deploy effective negotiation skills. The ability to apply advanced listening and facilitation skills The ability to communicate in a professional manner with members of the multi-disciplinary team.
