Job summary

The post holder will join an established team of practitioners who are part of an exciting service looking to improve discharge arrangements for patients, particularly those with complex needs requiring funded packages of care. We have 2 positions available: one in Norfolk and one in Suffolk.

This role will require the post holder to support clinical management and co-ordination of complex mental health discharge pathways, whilst working collaboratively within a wider multi-organisational team. You will work to developing processes as part of a co-produced service. It would be desirable for the post holder will require a working knowledge of commissioning processes, however in house training can be given. In the role you will use your experience and knowledge of discharge/rehab pathways out of hospital provision as well as needing to be influential regarding discharge decisions options. You will promote alternatives to restricted environments whilst working with the patient and the clinical team to increase confidence in the combinatorial impact of multi-agency discharge support, balancing risk, and responsibility.

The post holder will be expected to meet service delivery working Monday - Friday, 0900-1700

Main duties of the job

Responsibilities throughout the patient pathway

The tasks below will be integral to the complex patient pathway and processes in place to support their safe and sustainable discharge. Both pathways and processes are subject to change as the whole service continues to develop. The post holder will be expected to keep informed of wider network pathway developments.

On admission

Screen all admitted patients in conjunction with the ward staff using a screening tool in order to determine which patients present an indication of complex needs that will require further in-depth assessment to support appropriate discharge pathways. This task where possible will be completed in conjunction with social care colleagues.

Facilitate early discharge.

Complete comprehensive, holistic assessments for patients with complex mental health needs who are suitable for rehabilitation and will require a funded placement with external providers upon discharge from an inpatient ward. Use a range of assessment techniques including the routine use of standardised psychometric measures.

Interpret and structure assessment outcomes into meaningful formulations of needs and problems that are understandable to the patient, their carer and the wider multi-disciplinary team enabling joint decisions about future care management and delivery options. The wider multi-disciplinary team will include the patient's named nurse, ward consultant, key worker and social worker for example.
