An exciting opportunity has arisen for a Responsive coordinator at the West Suffolk Hospital. The successful post holder will work as part of an integrated team supporting the discharge to optimise and assess principles. They will need to be able to provide efficient, comprehensive coordination of services in collaboration with West Suffolk Foundation Trust, Community Health, Suffolk County Council and voluntary sector organisations.

You will be responsible for organising the daily coordination and early identification of patients who will benefit from discharge under the discharge to optimise and assess pathways. This will require the post holder to have a flexible approach and be able to work across all the services linked with the Responsive Service Team such as Medically Optimised Team, Support to Go Home, community assessment beds and pathway one.

You will need to undertake caseload management duties and provide person centred assessments as part of this role. You will also be responsible for the daily management of the Support to Go Home Reablement support workers. You will need to be a well organised person with excellent communication skills and enjoy working as part of a team. This post is developmental and will commence as Band 4 and on completion of competency framework over 18 months will move to Band 5.

This post will cover weekend working as it is a 7-day service and you will need to have the ability to travel to all rural destinations.

**Responsibilities**:
Clinical and Professional

Recognise barriers to discharge and develop problem solving mechanisms.

On a daily basis, review and coordinate the early identification of patients who will benefit from discharge under the Discharge to Optimise and Assess principles in order that the persons transfer from hospital can be planned and not delayed.

To have a strong focus in identifying patients appropriate for the responsive services screening patients and confirming with the ward MDT that the patient is medically optimised to transfer out to the community for assessment.

On a daily basis, review the medically optimised dashboard and screen reasons for delays and code appropriately within the medically optimised team protocol.

To provide day to day coordination of the Support to Go Home team when managing the daily caseload of the team, assisting the reablement support workers to allocate their caseloads and ensuring they have the right skills to provide a high quality service reporting any issues to senior members of the team.

To liaise with appropriate members of the multi-disciplinary team to unblock delays and improve patient flow within the West Suffolk Foundation Trust and community services in line with the discharge to assess principles.

Be confident liaising with patients and their preferred contacts that the patients meets criteria for the responsive services and is safe to go home. This will involve using own clinical reasoning or joint decision making with system partners.
