You will be working part of an established team of 5 Integrated Neighbourhood Teams across the borough and this team plays a crucial role in this. You’ll be leading health, primary care and social care professionals who will be working together as one team. You’ll help them work collaboratively with voluntary and community groups to empower people to take control of their care and improve well-being. It’s ‘devolution in action’: the people of Bury taking control of the way their own health and care services best meet their needs.

You will be the senior strategic lead for the development and implementation of Integrated Health and Social Care Neighbourhood Teams in each of the 5 Neighbourhoods of Bury.

**Main duties of the job**:
An exciting opportunity has arisen for an Allied Health Professional to support and develop the Active Case Management of complex cases across Bury. The post holder will be an autonomous practitioner who will be key to liaising with the wider AHP services across Bury and representing them at the 5 neighbourhood multidisciplinary meetings.

The post holder will be responsible for supporting the Active Case Management function of the Multidisciplinary Integrated Health and Social Care teams by offering expert advice, signposting and liaising with the wider networks as appropriate.

The post holder will lead in developing, delivering and evaluating high quality patient centred services in conjunction with the associated clinical teams to ensure this is incorporated within the Integrated Neighbourhood Teams and Local Care Organisation objectives

Realise the INT ambition at neighbourhood level as part of Together for Bury

The Neighbourhood Lead will be responsible for the effective delivery of integrated health and social care services which are neighbourhood based aligned geographically to GP practices.

To utilise an Organisational Development approach to bring together staff from multi professional and multi agencies to develop a high performing, cohesive Neighbourhood Team with the right values, behaviours and competencies

Develop and manage key relationships with GP practices and other key stakeholders to deliver the vision for INT
