Learning Disability Forensic Practitioner

Job summary
We have an exciting opportunity for a registered professional to join the Learning Disability Forensic Team as a band 6 practitioner- hours a week.
The team provide a service Monday- Friday 9am -5pm, the successful candidate will have the opportunity to discuss flexible working pattern within these hours.
You will be working within a small specialist team of nurses, health care support workers, Psychiatry and Psychology, with input from allied health professionals- which is part of the wider Learning Disability Service.
The Learning Disability Forensic Team are based at County Hall, Glenfield, Leicester and provide support, clinical assessments, interventions and risk management for people with learning disabilities presenting with forensic needs.
The service is provided to people living in the community of Leicester, Leicestershire and Rutland and additionally to patients who are currently in inpatient psychiatric settings supporting their discharge into the community and thereafter.
This post is an excellent opportunity for a registered professional (professional qualification and registration) who has some experience of working with people with learning disabilities and complex high risk / forensic needs to develop and strengthen clinical skills and competencies and additionally work within a team who are highly passionate in relation to supporting people with learning disabilities and complex needs to live successfully within the community.
Main duties of the job

Hold a small caseload of patients with complex needs living in the community, completing specialist forensic risk assessment, risk intervention and oversight Provide clinical assessment, support, planning and oversight for patients of whom are currently in psychiatric units with forensic needs preparing for a safe discharge and ongoing monitoring. Act as lead professional and completion of required assessments and record keeping Working collaboratively with inpatient teams, commissioners and other case managersSupport the Community Learning Disability Teams where patients are presenting with potential forensic risks- through consultancy and advice. Offer training and advice to carers, providers and health and social care staff specifically in relation to meeting the individuals forensic needs. Provide specialist advice and support to people to enable them to remain in the community and prevent, if possible, in-patient admissions. Where required the service also provide direct interventions.The successful candidate will receive a full induction to the Forensic Team and wider Learning Disability Service and further to this, access to bespoke training in relation to the assessment and risk management of patients with learning disabilities and complex behavioural and forensic needs.
You will need a full driving license and access to a vehicle insured to use for work purposes.
