We are seeking a highly motivated and experienced Disability Forensic Care Coordinator to join our dynamic team at Leicestershire Partnership NHS Trust. This role involves working with individuals with learning disabilities and complex needs, providing specialist forensic risk assessments, interventions, and oversight.

Key Responsibilities:

  • Hold a small caseload of patients with complex needs living in the community, completing specialist forensic risk assessment, risk intervention, and oversight.
  • Provide clinical assessment, support, planning, and oversight for patients currently in psychiatric units with forensic needs, preparing for safe discharge and ongoing monitoring.
  • Act as lead professional and complete required assessments and record keeping.
  • Work collaboratively with inpatient teams, commissioners, and other case managers to support the Community Learning Disability Teams where patients present with potential forensic risks.
  • Offer training and advice to carers, providers, and health and social care staff on meeting individuals' forensic needs.
  • Provide specialist advice and support to enable individuals to remain in the community and prevent in-patient admissions.
  • Where required, provide direct interventions.
