Job summary

This is a key role for Beverley PCN offering support to a wide rage of individuals in the Beverley PCN area.

We have patients with a wide variety of needs and this would be an exciting and challenging role in being able to identify how best to help and assist them.

Primarily placed within the GP Practices within thePCN, social prescribers enable people to manage and improve their health, well-beingand resilience by connecting them in to community services, groups, activitiesand other pursuits.

Main duties of the job

The role will require managementand prioritisation of your own caseload, in accordance with the needs,priorities and any urgent support required by the individual. It is thereforevital that you have a strong awareness and understanding of when it is appropriateor necessary to refer people back to other health professionals/agencies, whenthe individuals needs is be beyond the scope of this role.

You will support existing groups tobe accessible and sustainable and if required, help people to start newcommunity groups, working collaboratively with all local partners. This roletherefore requires some multi-agency working as determined by the PCNLeadership Team.

About us

Beverley PCN has been established for 5 years and has a wide variety of staff delivering additional services to aid the patients of the 4 practices within the Beverley PCN group.

The 4 practices are: Greengates Medical Group, Manor Road Surgery, North Beverley Medical Centre and The Old Fire Station Surgery. All 4 practices collaborate to give their patients access to the best services possible.

As the PCN social prescriber, you will be able to directly impact the patients of the PCN and will be supported to become a valued team member of all of the practices.

Use solution focussed techniques such asmotivational interviewing to give people the time and space to identify whatmatters most to me and to consider the impact of wider issues such as debt andpoor housing on their health and well-being.

Work with the person to coproduce a simpleand tailored action plan which is tailored to their priorities, interests andmotivations and is designed to overcome any barriers to success.

Maintain excellent community knowledge andundertake research to identify the best services, groups, activities, and otherpursuits for a persons specific needs and goals. Encourage people to take independent steps toimprove their well-being.

Facilitate new and innovative solutions tocomplex situations as well as common barriers faced by people using theservice.

Maintain an excellent knowledge ofappropriate personal and charity grants; facilitating access for people wherethis will help them achieve or sustain greater independence.

Be local leaders and advocates, promotingsocial prescribing to a wide range of partners and community agencies throughnetworking and attendance at relevant forums.Work with community organisations to identify local gaps, pressures, andpossible solutions.

Effectively manage a caseload of clients,ensuring that all quality and monitoring standards are met and that performanceinformation is provided within the required timeline.

Qualifications

Essential

Qualifications Essential Educated to GCSE or equivalent (GCSE Maths & English C or above) Demonstrable commitment to professional and personal development Training in motivational coaching, behavioural change and goal setting

Desirable

Qualifications Desirable NVQ 3 or equivalent and/or relevant basic/first level professional qualification, or working towards this Health and Fitness qualification L3, Coaching Qualification(s) / Mentoring L3, Health and Social Care L3 Diploma or working towards this

Experience

Essential

Understanding of the determinants of health to include social, economic and environmental factors Understanding of, and commitment to, equality, diversity and inclusion Knowledge of IT systems to include the ability to use word processing skills, emails and internet Experience in establishing and running health promotion groups. Ability to build and maintain relationships whilst maintaining appropriate professional boundaries Excellent verbal communication skills with the ability to communicate effectively at all levels inc. customers, patients and carers, specialist services, GPs and colleagues. Listening skills displaying empathy. Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences A current UK driving licence and access to a vehicle Good technical literacy of Microsoft Applications Word, Excel, and Access etc.

An appreciation of the nature of general practice Knowledge of the Personalised care approach Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement (can include unpaid work) Experience of supporting people, their families and carers in a related role (can Include unpaid work) Experience of supporting people with their mental health, either in a paid, unpaid or Informal capacity Experience of data collection and using tools to measure the impact of services Knowledge of community development approaches Knowledge of public health issues in the local area Experience of partnership/collaborative working and of building relationships across a variety of organisations including the voluntary sector Experience of using GP clinical systems, such as SystmOne / EMIS Experience with alcohol/ drugs intervention, smoking cessation training or experience in health and nutrition work with patients
