Job summary

Missing Link providespractical advice and support to individuals to allow them to maintain a rich andmeaningful life. The aim is to deliver recovery and wellbeing opportunities forpeople with mental health and other support needs to achieve their hopes andambitions.

As our Recovery Navigator, youwill work collaboratively together with the service user (male and female) andmental health professionals to navigate and coordinate services (both socialand clinical interventions), connecting into community resources, buildingresilience and improving wellbeing. Workwithin a recovery and psychologically informed approach, taking a holistic viewto support peoples recovery to live fulfilling lives. Ensure that services aretailored to individuals cultural and social needs.

Main duties of the job

Buildtrusting and collaborative relationship with the service user (male or female).Coproduce, implement and review recovery plan, crisis plan and otherself-management tools. Casemanage and co-ordinate support, referring and liaising effectively with allprofessionals, agencies and other parties involved in service users support inorder to deliver the support package. Workclosely with colleagues with the service and build strong partnership workingwith other agencies such as Health and Social Care, Voluntary and CommunityService. Support/ facilitate service users to access services. Maintainhope and optimism for the individual with high expectations for recovery. Uselived experience (if appropriate) to positively support the individualsrecovery. Must holda full driving licence and have access to appropriate motorised vehicle

The post is part of theAssessment and Recovery Team and involves working with and alongside multi organisations(Mental Health Bristol Partnership). The post holder will need to work collaborativelyand in partnership with all these agencies as one team.

Job Accountabilities

Build trusting and collaborative relationship with the service user.

Carry out role adhering to Mental Health Bristol Partnerships policies,procedures, values, code of conduct and current legislation using a Recovery orientated, psychologicallyinformed, equalities based approach

Co produce, implement and review recovery plan, crisis plan and otherself-management tools Wellness Recovery Action Plan, in partnership withservice users in line with best practice.

Case manage and co-ordinate support, referring and liaising effectivelywith all professionals, agencies and other parties involved in service userssupport in order to deliver the support package. Work closely with colleagueswith the service and build strong partnership working with other agencies suchas Health and Social Care, Voluntary and Community Service. Support/ facilitateservice users to access services.

Identify, assess, contribute, maintain, feedback and manage risk toensure risk is minimised. Work in a safemanner using a positive risk-taking approach, ensuring the health and safetypolicies of Mental Health Bristol are implemented and fully adhered to ensureall safeguarding issues are properly managed.

Have up-to-date safeguarding training and ensure that Safeguarding policiesand procedures are fully adhered to and that a Think Family approach isembedded within working practice.

Enable delivery of the recovery plan and to enable service users to makeinformed decisions about their mental wellbeing, maximising their independenceand providing advocacy role where appropriate. Build trusting and collaborativerelationship with the service user.

Work in a culturally sensitive way for all service users, tailoringservice to meet their individual needs, challenging stigma and discrimination,advocating for person if appropriate, and ensuring up-to-date community knowledge.Work with a range of needs from young people to older people, LGBT serviceusers, disabled people and with people with a range of backgrounds andethnicity.

To provide specialist support to women and men using the service and adviceand guidance to other team members regarding womens mental health issues

To provide specialist support and screening regarding domestic abuse/sexualviolence and provide advice and guidance to the team on pathways into DVA andsexual violence services.

Participate actively in supervision and reflective practice. Proactively seek advice and support from colleagues and other agencies as relevant

Promote and facilitate service user involvement and inclusion within MentalHealth Bristol and the community in line with Mental Health Bristol Partnershipobjectives.

Contribute to project work, development initiatives and implementationof policies and procedures with guidance from Team Manager and Partnership andRecovery Development Managers.

Manage caseload, documentation and time effectively, ensuring up to dateand clear record keeping in line with BMH policies, making best use ofsupervision, training and staff development. Ensuring up to date and clearrecord keeping in line with policies.

Collaborate with other team members to maximise service performance,meet targets and Service Level Agreements and promote effective communicationand teamwork.

Work within rota system including evenings and weekend working asrequired.

Contribute to Recovery College delivery and promote college activitiesto Service Users, family, carers and enable access.

Maintain hope and optimism for the individual with high expectations forrecovery. Use lived experience (if appropriate) to positively support theindividuals recovery.

Work closely with carers, families and children as with service userscarers if appropriate.

Person Specification

Knowledge, skills and experience

Essential

Numeracy and literacy to GCSE level/NVQ 2 or equivalent IT skills including ability to produce various documents in Word, and use of email and internet. Proven knowledge of support needs of people with mental health needs. Proven knowledge of equal opportunities issues Proven knowledge of health and safety issues, especially those relevant to mental health Proven knowledge of safeguarding policies and procedures relating to adults and children and young people Knowledge and understanding of the particular needs of working with vulnerable women A minimum of two years significant experience working with people with mental health needs, or people with complex needs Proven experience of working with people with multiple needs ( mental health, homelessness, alcohol/drug dependency long term conditions) Proven experience of working with victims of domestic abuse and sexual violence Proven experience of carrying out needs and/or risk assessments and the support planning process Experience of working with a number of individuals with competing needs and priorities Commitment to diversity and equal opportunities at work Work with a Recovery focused approach Commitment to service user participation and involvement Commitment to continued personal development An understanding and commitment to meeting the needs of vulnerable women A current, full driving licence and access to appropriate motorised vehicular transport (such as a car, moped or motorbike).
