With the advanced development of Primary CareHealth Networks (PCNs) in Tower Hamlets, Bromley by Bow & Stepney Health (BASH)CIC are looking for a highly motivated, innovative personto join our Primary care Network Team as an Experienced Healthcare Assistant with phlebotomy experience.
We are looking foran enthusiastic experienced Healthcare Assistant to join the PCN to support the catch up on long-term condition reviews after the pandemic andclear the backlog. The focus of this role is to support housebound and carehome patients who are vulnerable, have co-morbidities and complex health needs,which are not always met in a timely fashion.
Main duties of the job
To support practices(Bromley by Bow/XX Place, St Andrews, St Pauls Way, and Wellington Way) in theNetwork to achieve QOF, LES , and DES targets for patients thatare housebound and on the complex integrated care (IC) list. These patientswill have a named Care Co-ordinator, HCA and Clinician. They have complexhealth needs , are palliative, have dementia, live in care homes or arehousebound, and often, multiple long term conditions.
TheHCA will be fully trained, qualified and competent in Phlebotomy
The HCA will supportthe Primary Care Network(PCN) teams to catch up and maintain long-termcondition management post pandemic and to help the primarycare network (PCN) to achieve its targets for adult immunisation and cardiovascularhealth programmes.
The HCA will visit housebound,care home patients for routine monitoring who are vulnerable, haveco-morbidities, and complex health needs, which are not always met in a timelyfashion.
TheHCA will work collaboratively with the multi-disciplinary general practice teamto meet the needs of patients, supporting the delivery of policy andprocedures.

BASH CIC was formerly known as MEEBBB Health CIC, hasbeen established for 12 years, and employs a number of staff for memberpractices. In 2010 the thenPrimary Care Trust, Local Medical Council and GP surgeries in Tower Hamlets setup Networks composed of member practices linked to a Network Incentive Scheme(NIS) as a contract for additional work over and above their standard GPcontracts; this has now changed to the LES (Local Enhanced Services). Thereason for establishing the PCNs was due to the high levels of deprivation inthe borough, and to address the chronic underfunding in primary care

The Health CareAssistant will visit housebound IC patients and undertake care plan A,(phlebotomy, blood pressure monitoring, diabetes foot check, urinalysis,spirometry as required).
The HCA will input datato the clinical templatesThey will escalatewhen appropriate to the multidisciplinary team (social prescribers, clinicalpharmacist, GP, Nurse, community health services, to address thepatients needs)

Provide feedback tothe monthly IC multidisciplinary team meetings
Promote healthy livingand encourage uptake of preventative health promotion activities in theNetwork
Work in collaborationwith the care coordinator to plan visits in a timely and holisticmanner.
The PCN Clinical Leadto provide feedback and supervision to HCAs, Clinicians, and Care Coordinators.
