The following are the core responsibilities of the PCN Paramedic. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels. Clinical Practice Work as an autonomous clinician to provide expert clinical advice, support and care.

Work as part of proactive and collaborative multidisciplinary team (MDT) within the PCN. Support the PCN anticipatory care team with caseload management. Admit or discharge patients to and from the caseload, and refer them to other care providers internally and externally as appropriate, and in line with practice protocols. Clinically assess and triage patients, including same day triage, and as appropriate provide and review interventions/treatments (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.

Consultations may be conducted by home visit, face-to-face, video, or by telephone. Develop and utilise clinical skills to analyse and interpret history of illness, presenting symptoms and physical findings to enable diagnosis, proactive care planning and treatment of patients. Perform specialist health checks and reviews within scope of practice and in line with local and national guidance. Undertake the collection of pathological specimens, including venous blood samples and swabs.

Perform and interpret ECGs alongside other results as appropriate. Perform investigatory procedures needed by patients and those requested by GPs and the wider PCN. Assist with anticipatory care-planning and the care of patients at end of life, ensuring care plans are put in place and actioned as needed. Prioritise and co-ordinate the multiple health care needs of the patient, co-ordinating and facilitating timely referrals to other team members within acute, mental health and community service settings.

Work with patients in order to support compliance with, and adherence to, prescribed treatments and medications. Ensure all patients who may be vulnerable, or who have mental health needs, are appropriately managed and referred on to the necessary mental health team. Participate in MDT processes and pathways, review identified patients, conduct face-to-face assessments and participate in MDT meetings, virtual wards, discharge planning meetings and ward rounds, as required. Communication Communicate proactively and effectively with colleagues across the multidisciplinary team.

Advise patients appropriately and provide onward referral if they require the services of other primary or secondary healthcare professionals. Utilise and demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment. Communicate effectively and sensitively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating. Act as an advocate for patients and colleagues.

Ensure awareness of sources of support and guidance, and provide information in an acceptable format to all patients. Liaise with other stakeholders as needed for the collective benefit of patients. This is including, but not limited to: Patients GPs, nurses and other practice staff in the PCN Other healthcare professionals PCN clinical directors Care homes Community nurses and other allied health professionals Community and hospital pharmacy teams Contribute to internal and external stakeholder meetings. Analyse, evaluate and present service data and patient outcomes as required by the PCN.

Provide assurance to the PCN Executive Team that contractual requirements are being complied with. Quality and Safety Comply with the Health and Care Professions Council (HCPC) Code of Conduct, and keep registration up-to-date, documented and checked within the service on an annual basis. Be verified against the paramedic primary care roadmap. Produce accurate, contemporaneous and complete records of patient consultations, consistent with legislation, policies and procedures.

Provide care that is evidence-based, working to agreed and established NSF and NICE clinical guidelines, and support others to do the same. Prioritise, organise and manage own workload in a manner that maintains and promotes quality. Recognise and work within own competencies and refer to a more appropriate colleague(s) when necessary. Be responsible for implementing, monitoring and training of infection control policies and procedures according to local and national guidelines.

Undertake training needed to ensure competencies for delivering all responsibilities and enhance own performance through continuous professional development. Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation. Initiate and participate in the maintenance of quality
