Are you looking to make your career in the field of Diabetes. We are looking to recruit a full-time B5 Diabetes Nurse. You will be supported by a team of Diabetes Specialist Nurses, Diabetes Medical Consultant, and a Diabetes Nurse Consultant.

The post holders will develop a structured approach to the management of Diabetes patients, conduct Diabetes Risk Assessment health checks, deliver Structured Education programme - DESMOND and will work to agreed clinical protocols and guidelines ensuring comprehensive care across the diverse population of the Borough.

You will have the willingness to learn and develop into a Specialist Diabetes Nurse.

PCN care and education coordinator plays an important role within the PCN by adopting a flexible approach to patient care and service delivery. The aim is to help patient to improve their quality of life.

- The post holder will be responsible for the coordination, planning, implementation and evaluation of the diabetes education as well as providing clinical support to the patients with Harrow locality.
- The postholder will be working collaboratively with GP’s within Harrow PCN and the Community Diabetes Specialist Service.
- The post holder will be a registered nurse and work in line with the requirements set out in the NMC code which require all registered nurses to prioritise people; practice effectively; preserve safety; promote professionalism and trust.
