**JOB TITLE**: Health Care Assistant (HCA)

**REPORTS TO**: Operational Manager

**HOURS**: 15 - 25 hours per week, with opportunity to work in extended hours hubs on Saturday. There could also be opportunity for full-time hours.

**SALARY**: £11.95 per hour

**CONTRACT TYPE**: Permanent

**LOCATION**:Northern Medical Centre, Barnsbury Medical Practice

Extended access hubs across Islington, as appropriate

The Role:
Barnsbury Medical Practice (BMP) and Northern Medical Centre (NMC) are managed by Islington GP Federation (IGPF). Islington GP Federation (IGPF) is looking for 2 Health Care Assistants for an innovative position working across these practices.

The post holder will be a key player to drive the North Central London Long Term Conditions LCS.

You will have a role in work being undertaken by the team including:
Ø Call and recall systems

Ø Receiving experience with wound care and venepuncture

Ø Aiding follow ups

Ø Conducting physical health checks, including:

- Phlebotomy
- Peak flow readings
- Blood pressure
- Weight & lifestyle questionnaires

Ø Checks for metabolic diseases, including:

- Diabetes
- Chronic kidney disease
- Hypertension

Ø Checks for respiratory conditions, including:

- Asthma
- COPD (Chronic obstructive pulmonary disease)

There will be training available to help you develop your role.

There will also be opportunities for professional development into a trainee nursing associate role if this is something you would be interested in.

**About Islington GP Federation**

Islington GP Federation (IGPF) is a growing organisation representing 31 practices; we have established ourselves as a leader in new ways of working, including running Islington’s extended access primary care services (I:HUB) as well as supporting the Islington Primary Care Networks (PCNs). Our current range of services include the Extended Access Service, I:HUB, Community Ear, Nose and Throat (ENT), Integrated Community Gynaecology, practice-based pharmacists and a range of practice support mechanisms.

IGPF works very closely with a range of partners including the regional commissioning group, NHS England, Healthy London Partnership, Public Health, local hospitals such as UCLH, the Whittington Health and the London Borough of Islington.

IGPF is the host organisation for the Primary Care Network (PCN) workforce and the Islington Training Hub and has been working for over two years to create training and development programmes that meet the needs of staff working in primary and community care settings. IGPF runs two Islington GP practices on caretaking contracts; a third practice will be taken on within this financial quarter.

**Responsibilities**:
Ø Request and follow up blood tests

Ø Perform wound care and venepuncture

Ø Record and monitor vital signs, recognise abnormalities, and escalating to a senior when needed.

Ø Perform ECG recording.

Ø Maintain accurate records of patients’ conditions, care received and given

Ø Handle enquiries, concerns and complaints from patients and visitors in a professional manner

Ø Conduct physical health checks; Phlebotomy, peak flow readings, blood pressure, and weight & lifestyle

Ø Conduct checks for metabolic diseases; diabetes, chronic kidney disease, and hypertension

Ø Conduct respiratory conditions’ checks; asthma and COPD

Ø Contribute to discharge process

Ø Assist with preparation of clinical procedures and clearing environment afterwards as appropriate.
