Job description

Job responsibilities

To be responsible forthe assessment of newly referred adults who may be undergoing their firstpsychotic episode. To work with diagnostic uncertainty, providing ongoingcomprehensive assessment and outcome monitoring of all caseload service users.

To assertivelyoutreach difficult to engage service users. To meet service users regularly, torespond flexibly when agreeing venue, such as home visit, team base, education,work or recreational establishment to assist with engagement & encourageparticipation in treatment delivery as well as facilitating independence

To involve familiesand carers in treatment plans where appropriate. This may require direct FamilyIntervention work. Ensuring services are provided within guidance of theChildren & Families' Act and Care Act.

To work inco-operation with members of the multi-disciplinary team and all other relevanthealth and social care professionals to assess, develop, implement and evaluatehigh quality packages of client-centred care.

Experience with working with people of the age group of 18-65 MDT experience Knowledge of trust recording system (ePJS) and knowledge of local services beneficial
