We have a number of exciting opportunities due to promotions in the team for experienced, hardworking Medicines Management Pharmacy Technicians to join our Medical and Surgical teams.

We are seeking highly motivated individuals with a passion for patient-centred care who will work closely with members of the wider multidisciplinary team to improve patient flow.

The successful applicant will need to be committed to self and service development, have excellent practice with meticulous attention to detail and be a dedicated team member.

Main duties of the job

The posts will provide a ward based medicines management service including comprehensive and timely medicines reconciliation, contributing to the safe, effective discharge of patients and the completion of audits. You will be pivotal in ensuring our patients get the right medicines at the right time by working with the wider multidisciplinary team and pharmacy service though effective communication and detailed handover.

The successful candidate will need to be self-motivated and organised and will be required to contribute to the process of service development and innovation. The post holder will be given a high degree of autonomy and must be able to work effectively alone and as part of a team.

As an advocate for pharmacy services and a guardian of medicines safety, applicants must be excellent communicators with good interpersonal and relationship building skills. You will also help in the training and development of our apprentices, assistants, pre-registration pharmacists and technicians and will be expected to undertake line management duties.

This role will be based at the Leicester Royal Infirmary; however, the successful candidate may on occasion be required to work at other sites.

The post holder will be expected to contribute to core pharmacy services including late, weekend, bank holiday and on-call cover on a rota basis.

Job responsibilities

To coordinate medicines supply activity at ward level and to provide effective communication and coordination with central dispensaries. To be responsible for the supply of items for one-stop dispensing (once clinically checked by pharmacist). This will include generating the order, dispensing and placing in the patient's bedside medicine cabinet and endorsing charts as per departmental policy. To ensure all patients medication is transferred to the correct ward with the correct patient as necessary. To implement and maintain the Patients Own Drug (POD) scheme for the designated area (and self-administration where introduced) according to trust-wide policies To coordinate discharge planning and implementation in conjunction with the clinical pharmacist and ward staff, through involvement with medical and nursing staff identifying patients for discharge, checking medicines against the TTO letter and dispensing, re-labelling as necessary. To ensure that accurate drug histories are obtained for identified patients by communication with the patient/carer and/or by liaising with external organisations including GP practices, community pharmacies and nursing/residential homes. To undertake patient counselling on selected patients and to work with the clinical pharmacist to ensure patients understand their discharge medication To liaise with ward staff over any relevant medicines management issues to ensure concordance/compliance with prescribed medication.
