Job summary

East and Central Brighton& The Deans and Central Brighton PCNs require clinicians able to workautonomously within the community, and out in the neighbourhood but fullysupported by a senior clinician on behalf of 14 GP practices in our PrimaryCare Networks. They will use their clinical assessment and treatment skills, toanswer requests from GPs and then prioritise and provide first line clinicalcare for housebound patients presenting with problems relating to illness orinjury that are not emergencies but might be of an urgent nature, or othermedical matters that can only be dealt with in a patients own home. You willbe part of a home-visiting team serving patients across a defined part of East andCentral Brighton.

Main duties of the job

We are seeking a dedicated and experienced Home Visiting Paramedic to join our Primary Care Network (PCN) home visiting team. This vital role involves organising and conducting home visits to provide comprehensive assessments, definitive treatments, and necessary referrals to other primary care, secondary care, or multi-disciplinary community teams.

Key Responsibilities:

Organise and arrange home visits for patient assessments and treatments. Conduct comprehensive clinical examinations, including physical and mental health assessments. Make diagnostic decisions based on clinical findings and interpret laboratory or imaging results. Prioritise health problems and intervene appropriately in complex and urgent situations, including initiating emergency care in conjunction with emergency services. Provide advice to patients on general healthcare and promote self-management, including signposting to community or voluntary services. Communicate effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required. Strong diagnostic skills with the ability to interpret clinical findings and test results. Ability to prioritise and manage health problems in complex and urgent situations. Excellent communication skills to work proactively and effectively with multi-disciplinary teams. Commitment to patient care and promoting self-management.

About us

The East and Central BrightonPrimary Care Network consists of pharmacists, social prescribers, care co-ordinators,physiotherapists, patient engagement workers, occupational therapists, mentalhealth practitioners / support workers, health and well-being coaches, nurses and paramedics all staffing severalmulti-disciplinary teams.

Weaim to improve local population health and breakdown barriers to primary care for patients and community services. In doing sowe believe primary care will become more accessible and rooted in our local communities. We see thisas a crucial step towards de-medicalising distress caused by poverty,isolation, and social fragmentation.

The PCN team is currently based in an office atcentral Brighton but individual staff members establish working patterns thatmeet the requirements of role, working at home, in GP practices or in thecommunity as appropriate.We take the wellbeing of our staff seriously with opportunities such as hybridworking and yoga session at work.

You willbe part of a home-visiting team serving patients across a defined part of East andCentral Brighton. The team will provide clinicalhome visiting between am and pm alongside clinical triage,coordination and administrative work. Essentialduties and responsibilities: 1. Assess and triage requests to visit patientsat home, organise and arrange home visits, and attend as appropriate to provideassessment and definitive treatment or make necessary referrals to othermembers of primary care, secondary care or the wider multi-disciplinarycommunity teams. 2. Whenattending a patient, assess by taking and recording a patients historyincluding medical, family and social history, talking to the patient, relativesor other carers as appropriate. Conducting a comprehensive clinical examinationof the patient including a physical examination of all appropriate systems ormental health assessment as clinically indicated and make diagnostic decisions based oninterpretation of clinical and other findings such as laboratory or imaging results. 3. After diagnosis, plan, implement andevaluate treatment/interventions and care for patients, often with complexneeds. 4. Some of this will be possible autonomouslyand some will need to be in conjunction with the wider Primary Care team 5. Prioritise health problems and interveneappropriately to assist the patient in complex and urgent situations, includinginitiation of effective emergency care if the need arises, usually inconjunction with the emergency services. 6. Advise patients on general healthcare andpromote self-management where appropriate, including signposting patients toother community or voluntary services. 7. Collaborate with other members of the PCNincluding doctors, nurses and other AHPs, accepting referrals where appropriateand referring to them for specialist care 8. Communicate effectively at all levelsacross PCNs and other organizations, ensuring safe & effective,patient-centred service 9. Communicate proactively and effectivelywith all colleagues across the multi-disciplinary team, attending andcontributing to meetings as required 10. Maintain accurate and contemporaneous health records appropriate to the consultation,ensuring accurate completion of all necessary documentation associated withpatient health care Desirableduties and responsibilities, which we would support training to develop if notalready competent in: 11. Be ableto: perform specialist health checks and reviews perform and interpret ECGs; alongside other results asappropriate perform investigatory procedures asrequired undertake the collection of pathologicalspecimens including intravenous blood samples, swabs, etc. perform investigatory procedures needed bypatients and those requested by GPs and the wider PCN 12. Prescribe, issue and review medications asappropriate following local and national guidelines and local care pathways. This will be done inaccordance with evidence-based practice and national and practice protocols,and within scope of practice 13. The overall objective is to provide analternative model to urgent and same day home visits for the PCN so we wouldaim to be able to demonstrate effective and efficient improvement by undertakingclinical audit and evaluation Essential QualityRequirements 14. Maintain registration withrelevant body or organisation and work within all legal, ethical andprofessional frameworks established by NMC/GMC/HCPC 15. Be capable of working at or work towards advanced practice and promoteevidence based practice. Consult local and national guidance on any relevantclinical topic where possible and appropriate, whilst being able to depart fromguidelines when necessary after discussion and agreement with the wider teamand patient. 16. Enhance own performance through continuousprofessional development, keep up to date withcurrent evidence-based practice, and impart own knowledge and skills toPCN colleagues to meet the needs of the service 17. Recognise and work within owncompetence and professional code of conduct as regulated by the HCPC, College of Paramedics, NMC & GMC 18. Assess effectiveness of caredelivery through self and peer review, benchmarking and formal evaluation 19. Participate in research andutilise the audit cycle as a means of evaluating the quality of the work ofself and the team, implementing improvements where required 20. In partnership with otherclinical teams, collaborate on improving the quality of health care respondingto local and national policies and initiatives as appropriate Administrativerequirements 21. Produce accurate,contemporaneous and complete records of patient consultation, consistent withlegislation, policies and procedures 22. be aware of data protection(GDPR) and confidentiality issues particularly within a PCN 23. use technology andappropriate software as an aid to management in planning, implementation andmonitoring of care, presenting and communicating information 24. review and process data usingaccurate read codes in order to ensure easy and accurate information retrievalfor monitoring and audit processes. Supervision & Support The postholder will have access to appropriate clinicalsupervision and an appropriate named individual in the PCN to provide generaladvice and support on a day to day basis. Through annual appraisal, we would encourage a policy ofactive training and development with significant leadership opportunities andcareer progression if the service continues to evolve to cover the whole cityand beyond. This will be a collaborative model of training led by the candidate,identifying training needs, and the PCN providing resource and support wheretraining has been agreed as relevant to the role. Experience: Proven experience in a paramedic role, including experience in home visits or community healthcare settings. Experience in conducting comprehensive clinical assessments and making diagnostic decisions. Experience in managing complex and urgent health situations, including the initiation of emergency care. Experience of dealing with vulnerable patients Skills and Abilities: Excellent clinical skills, with the ability to conduct thorough physical and mental health examinations. Strong diagnostic skills and the ability to interpret clinical findings and test results accurately. Ability to prioritise health problems and manage them effectively in complex and urgent situations. Strong interpersonal and communication skills, with the ability to work effectively with patients, families, and multi-disciplinary teams. Ability to provide clear and concise advice to patients on general healthcare and promote self-management. Effective organizational and time-management skills, with the ability to plan and conduct home visits efficiently. Knowledge: In-depth knowledge of primary and secondary care services, including referral pathways. Understanding of the roles and functions of multi-disciplinary community teams. Knowledge of relevant healthcare policies and procedures, including patient confidentiality and safeguarding. Personal Attributes: Compassionate and empathetic approach to patient care. Ability to work independently and as part of a team. Resilience and the ability to remain calm under pressure.
