About Our Organization:
The East and Central Brighton Primary Care Network comprises a diverse team of healthcare professionals, including pharmacists, social prescribers, care coordinators, physiotherapists, and mental health practitioners. Our mission is to enhance community health and improve access to primary care services, addressing barriers that patients may face.

Our team operates from a central location, with flexible working arrangements that allow staff to fulfill their roles effectively, whether from home, in community settings, or within GP practices. Job Responsibilities:
- Assess and triage patient requests for home visits, organizing and executing visits as necessary to provide comprehensive evaluations and treatments.
- Gather and document patient histories, conducting detailed clinical examinations and making diagnostic decisions based on findings.
- Develop, implement, and evaluate treatment plans for patients with complex health needs.
- Work autonomously and collaboratively with the broader Primary Care team as required.
- Respond to urgent health issues, providing timely interventions and coordinating with emergency services when needed.
- Advise patients on health management and connect them with relevant community services.
- Collaborate with other healthcare professionals within the network, accepting and making referrals as appropriate.
- Maintain accurate health records and documentation in compliance with legal and organizational standards.
Essential Qualifications:
- Registered Paramedic with the Health and Care Professions Council (HCPC).
- Relevant paramedic qualification (e.g., BSc in Paramedic Science or equivalent).
- Qualified in Phlebotomy.
Desirable Qualifications:
- Additional certifications in relevant areas, such as Advanced Clinical Practice or Mental Health First Aid.
