About the Role

We are seeking an experienced Therapist/Allied Health Professional to join our Admission Prevention Team at the Royal Sussex County Hospital. As a key member of our team, you will play a vital role in supporting the A&E department in preventing hospital admissions and ensuring seamless, safe, and effective discharge plans for patients.

Key Responsibilities

  • Work closely with A&E staff to identify opportunities for preventing hospital admissions and develop effective discharge plans.
  • Utilize your clinical expertise to support A&E staff in managing discharging patients into the community and community beds.
  • Support with the induction of new staff and provide training and education to A&E staff on community services and discharge management.
  • Attend MDT meetings and contribute to the development of effective discharge plans.
  • Ensure records and data are maintained in a timely manner and in line with information governance and data sets required for the service.

Person Specification

  • Professional qualification in Physiotherapy or Occupational Therapy.
  • Current registration with the Health and Care Professions Council as a Physiotherapist or Occupational Therapist.
  • Substantial relevant post-qualification experience covering a broad range.
  • Experience of working within a relevant specialist area and interdisciplinary working.
  • Experience of supervising junior staff, assistants, and students.
  • Experience of independently managing a clinical caseload and contributing to audit and evaluation within a clinical governance framework.
