We are looking for a highly motivated and autonomous therapy assistant to be part of our UCR team. This service provides holistic assessments, treatment and support to patients in their usual place of residence who are experiencing a health or social care crisis who might otherwise be admitted to hospital or require ambulance call out. You will be part of a service in which care is provided by a multi-skilled team.
The UCR service is a fast paced, interdisciplinary community team aimed at preventing unnecessary admission to hospital by visiting patients in their own homes and assesses the patient's safety to remain out of the acute environment. Patients are aged over 18 and are likely to present with frailty, falls or health deterioration. The team ensure the patient is supported out of hospital for a minimum 48 hour period and ensure transferred to appropriate services on discharge.
Main duties of the job
You will play a key role in supporting the UCR team in delivering high quality patient centred care to support individuals to remain safely at home. You will be working with a full multidisciplinary team of ACPs, physiotherapists, nurses, paramedics, occupational therapists, assistant practitioners and rehabilitation assistants.
You will also be part of our falls response service which aims to support patient's who have fallen at home and provide a falls pick up response and supporting with a holistic assessment of the individual's needs to support them to remain safely at home.

Job description
Job responsibilities
To work as an integrated member of the Urgent Community Response Service delivering person centred care, assisting Occupational Therapists, Physiotherapists, Nurses, Paramedics and the wider multidisciplinary team with patient interventions.
To deliver clinical interventions defined within individualised patient care plans, as directed by the senior staff.
To maintain high quality standards in all aspects of care delivery, working in partnership with primary care colleagues, other health professionals, patients, carers and the statutory, independent and voluntary organisations.
