An exciting opportunity is available to become part of our Breast Care team at Southmead and Weston as an Advanced Clinical Practitioner (ACP).

The job is an ACP band 8A but willing to take anyone interested in undertaking the trainee ACP as band 7. Please see the detailed Job descriptions for both.

The role involves autonomous, independent clinical practice. With a background of Nursing or Allied Health Professionals (AHP), the post holder will have completed training to practice as an autonomous Advanced Clinical Practitioner (ACP).

This will have incorporated the progressive development of history taking, clinical examination, clinical investigations, and interpretation and subsequently the diagnosis and management planning of the patient specific to Breast care.

To be responsible for the rapid assessment, diagnostics, and treatment of individuals within the Breast Surgery service using an advanced level of professional accountability, autonomy, and judgement, underpinned by Master’s level theory and experience. 

Main duties of the job

·Assess, differentially diagnose, plan, implement and evaluate individual treatment interventions for patients by virtue of expert clinical judgement and close liaison with members of the MDT.

·Exercise professional judgement to manage risk appropriately, especially where there may be complex and unpredictable events and supporting teams to do likewise to ensure safety of individuals, families and carers.

·Ensure compliance with their respective code of professional conduct and work within their scope of practice.

·Works in conjunction with the Breast MDT to clearly define values, direction and policies impacting upon care.

·Work in partnership with individuals, families and carers, using a range of assessment methods as appropriate.

·Be able to demonstrate a critical understanding of their broadened level of responsibility and autonomy. 

·Be responsible and accountable for their decisions, actions and omissions at this level of practice.

·Demonstrate critical reflection on own practice, self-awareness, emotional intelligence, and openness to change.

·Demonstrate effective communication skills, supporting people in making decisions, planning care or seeking to make positive changes, using Health Education England’s framework to promote person-centred approaches in health and care.

Detailed job description and main responsibilities

·Act as a clinical role model/advocate for developing and delivering care that is responsive to changing requirements, informed by an understanding of local population health needs, agencies and networks.

·Actively implement effective systems and processes for infection control management whilst working clinically and be a role model for other members of the clinical team.

·Initiate drug therapy/medication within the parameters of agreed clinical guidelines and in accordance with the legal framework you work under to supply and prescribe medicines (if from professional group legally entitled to prescribe.

·Evidence of post graduate Masters level in Advanced Practice, to include breast examination, Physical assessment and diagnostic reasoning.

·Further Specialist training in Breast care.

·Communication skills training at level 2 or equivalent.

·Role model the trust values, demonstrating a person-centred approach to service delivery and development.

·Contribute and where possible lead new practice and service redesign solutions in response to feedback, evaluation and need, working across boundaries and broadening sphere of influence.

·Critically apply advanced clinical expertise to influence clinical practice to enhance quality, reduce unwarranted variation and promote the sharing and adoption of best practice.

·Evaluate their own practice, and participate in multi-disciplinary service and team evaluation, demonstrating the impact of advanced clinical practice on service function and effectiveness, and quality.

·Continually develop practice in response to changing population health need, engaging in horizon scanning for future developments

·Pro-actively initiate and develop effective relationships, fostering clarity of roles within teams, to encourage productive working.

Active professional registered qualification with a professional body eg: NMC, HCPC MSc in advanced clinical practice Evidence of ongoing professional development through maintenance of professional portfolio and registration revalidation Excellent organisation and leadership skills A sound understanding of professional priorities and issues

Msc in advanced clinical practice - desirable for trainee ACP applicants

The successful applicant(s) will normally commence at the minimum of the scale unless they have previous NHS service at the same band. Progression through the scale is by annual increments.
