Job summary

We are looking for a Nurse Associate or Health Care Assistant to join our friendly busy Nursing team.

If you have experience in this area we are looking for an enthusiastic and motivated Nurse Associate or HCA to join the team.

Hours for this role will be 22 hours per week over 3 days. Working days will be Tuesday, Thursday and Friday. Thursdays will be - with flexibility on start and finish times on the other 2 days.

Main duties of the job

To be responsible for the delivery of the Practice's Nursing Services working as part of a multi-disciplinary team ensuring provision of high standard of patient care.

To work closely with all clinicians and admin staff to ensure accuracy and safety of procedures and working practice.

Job responsibilities

Carry out routine treatment room duties as delegated by a senior colleague.

Carry out annual reviews delegated by a senior colleague as appropriately trained.

Assist and perform routine tasks related to patient care as directed by senior nursing staff and GPs.

Follow agreed protocols and PSD's with referral to senior nurses or GP's.

Access, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being.

Signpost to social prescribers, wellbeing team and other health promotion teams as appropriate.

Demonstrate a good understanding of the Mental Capacity Act/Mental Health Act/Deprivation of Liberties and apply principles to everyday practice, seeking guidance from a senior colleague as required.

Carry out Mental Health reviews, working alongside other staff as needed.

Recognise symptoms for general psychiatric disorders.

Dementia awareness, promote staff awareness, patient information and local services.

Demonstrate engagement with carers.

Document consultations according to NMC/Practice guidelines.

Carry out Learning Disability reviews, working alongside other staff, as needed.

Demonstrate a good understanding of the principles of consent and ensure valid consent is obtained prior to undertaking any nursing care and procedures.

Ensure awareness of statutory and local clinical adult and children safeguarding protection procedures, including systems of referral. Ability to recognise signs and symptoms of child abuse.

Ensure all mandatory training is completed on an annual basis including safeguarding.

Experience

Essential

Relevant clinical experience according to responsibilities as per advert and Job Description. Excellent interpersonal and communication skills. Ability to prioritise and manage workload. Ability to work as part of a team member and autonomously. Ability to follow policy and procedure.

Desirable

Nursing Associate qualification. Care certificate. Knowledge of Primary Care. 2 years plus experience in a clinical setting. Experience of using TPP (SystmOne electronic medical recording system).
