An exciting opportunity has arisen for a band 5 Care Cordinator to join the Stamford Adult Community Mental Health Team. Nurses, Social Workers and Occupational Therapists are all welcome to apply.

We are a friendly, supportive and proactive team with a culture of high standards of care and any successful applicant will need to be committed to maintaining this. The team works closely with a wide range of internal and external partner’s including GPs, social care, Neigbourhood Teams, Place Based Teams.

You will also work alongside carers, ensuring that their views are taken into consideration.

You will need to be organised, flexible and committed to delivering the highest possible care to our client group.

The MDT is made up of Medical, Nursing, Occupational Therapy and Psychology colleagues and we highly regard one another's professional background and contribution to the care of our patients. We view supervision as fundamental to the functioning of the team both clinically and in terms of the wellbeing of our staff. We can provide professional supervision and CPD opportunities both profession specific and more generally.

The teams base is the Stamford Resource Centre. Core working hours are Monday-Friday 09:00-17:00.
