**The Role**

Our client is seeking an experienced and resourceful Care Professional with great planning skills to play a key role in their OUTSTANDING team.

Are you kind, compassionate and caring? Are you an experienced Care Professional with great IT, planning and interpersonal skills? Have you been involved in managing rotas? Then the role of Care Coordinator might just be right for you.

Reporting to the Registered Care Manager, your role will be to ensure that client visits are planned and carried out reliably and to the highest standard.

**Responsibilities**
- Creating and optimising care rotas
- Liaising with clients, their families and their Care Professionals
- Supporting the delivery of the highest standards of care
- Maintaining continuity of care calls for clients and carers
- Carrying out care calls where needed
