We have an exciting opportunity for you to join the Mid Notts Aging well Team, we are looking to recruit someone who is a good communicator, a team player and can work as part of a team is enthusiastic, well organised, and a highly motivated individual You will be committed to promoting independence and working in a strength-based way. You will primarily be working with adults, covering the Ashfield South and Hucknall area of Nottinghamshire.

This is an excellent opportunity for someone wanting to work within Adult Social Care, and Health with statutory and voluntary organisations in the community, to further develop multidisciplinary working skills. The role involves undertaking social care assessments, using the three conversations approach. Identifying and maximising opportunities to promote a person’s independence.

You will have an understanding of the Mental Capacity Act 2005, Best Interests, and relevant Adult Social Care legislation, including the Care Act 2014. You will be offered regular formal and informal supervision, and 6 monthly employee personal development reviews, a four-week induction to further support you in the role, and access to all relevant and appropriate training. We have a flexible hybrid working model, you must hold a full driving licence and have access to transport is essential.
