This is an exciting opportunity for a full time Community Care Officer to join an energetic and dynamic Discharge to Assess Team, working across Nottinghamshire.
We are a Multi Disciplinary team of OT's CCO's and Social Workers, who are committed to ensuring that people are discharged from hospital safely, with the right support at the right time, ensuring any ongoing rehabilitation needs are identified to enable people to live as independently as possible. We form part of the Acute/sub-acute discharge services in South Notts.
Acute/sub-acute services consist of the Integrated Discharge Team (IDT), which is a hospital-based service, and Discharge to Assess Team (D2A) that provide follow up support in the community. This vacancy is within the IDT team, however, the you will be required to work flexibly during times of need.
You will be required to assess need via a conversational approach, commission services and monitor packages of care within set timescales. You must have knowledge of community care legislation such as The Care Act 2014, Mental Capacity Act 2005, Carers Act, etc and relevant policies and procedures, and be committed to providing a high-quality service. Ongoing training is provided and is an essential aspect of this role.
You will contribute to the continuous improvement of the service incorporating Strengths Based Approaches, evidence based assessments and outcome focused personal budgets. We work in partnership with our colleagues in Health to promote holistic assessments and support/discharge planning, and with people and their Cares and Families to foster a Strengths Based Approach.
The ability to communicate verbally and in writing with service users, carers and other professional colleagues is essential.
The ability to work some weekends and bank holidays is a requirement of this role - weekend working is on a rotation basis of about one Saturday every 2 to 3 months.
