Community Care Officer

This is an exciting opportunity to join the Gedling Ageing Well team where we have various full time, part time, permanent and temporary posts available.
As a valued member of this team, you will be required to commission services and monitor packages of care within set timescales. You must have knowledge of community care legislation such as The Care Act 2014, Mental Capacity Act 2005, Carers Act, etc and relevant policies and procedures and be committed to providing a high-quality service. Ongoing training is provided and is an essential aspect of this role.
You will need to be enthusiastic, motivated, and organised in your approach to deliver a quality service and committed to strength-based practice, promoting independence and wellbeing whilst safely managing risks.
We work closely with our colleagues in Health to promote holistic assessments of need, and jointly with people, their Cares and Families to foster a partnership way of working within a Strengths Based Approach.
You will be working in an exciting and sometimes challenging environment in close partnership with external agencies and providers, which will require high levels of confidence and independence.
You will need to have effective communication, negotiation, organisational and IT skills which are essential tools to aid developing flexible working practices.
The team has adapted hybrid working, where you will be partly office and partly home based.
In return, you will receive support and training to help you reach your potential. We will provide regular formal and informal supervision.
